Position Summary

The Clinical Educator – Home Health is responsible for modeling the 3 Company values of Compassion, Integrity, and Excellence, and for promoting the Compassus philosophy, using the 6 Pillars of success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the Company, among all departments, and all external stakeholders. The Clinical Educator – Home Health implements onboarding education for new colleagues across programs or markets as well as ongoing educational needs to support quality of care provided to home health patients, families, and staff. S/he assists the National Director of Education in development and implementation of education. S/he implements and manages education schedules and evaluates new colleagues' progress throughout onboarding. The Clinical Educator is responsible for applying rules and regulations of state and federal regulatory agencies and other certified agencies.

Position Specific Responsibilities

  • Supports agency and Corporate policies, goals and objectives.
  • Promotes a positive working relationship between agency and Corporate colleagues.
  • Attends and participates in staff meetings, as requested.
  • Promotes homecare philosophy.
  • Communicates identified needs and potential solutions to supervisor.
  • Stays informed of current Medicare/Medicaid regulations.
  • Provides education to support clinical charts meeting Medicare/Medicaid regulations.
  • Evaluates to ensure effectiveness of educational/onboarding content and delivery, and assist in developing strategies to correct deficiencies.
  • Manages onboarding implementation for new colleagues across programs or markets and supports ongoing clinical education as requested.
  • Implements and manages education schedules and evaluates new colleagues’ progress throughout onboarding. If updates are needed to meet the needs of new colleagues and/or Programs, Clinical Educator will manage such updates to ensure needs are met.
  • Ensures audit of colleagues’ documentation at regular intervals throughout onboarding to assess for quality and compliance.
  • Tracks to ensure completed Competency Checklist is obtained prior to colleague’s release to new role.
  • Ensures effective delivery and successful completion of educational onboarding to positively impact quality care, outcomes and patient satisfaction, as well as colleagues’ satisfaction with onboarding experience.
  • Accepts responsibility for positive onboarding experience for all new Home Health colleagues and Leaders to ensure satisfaction and retention, decreasing turnover across Agency.
  • Reviews documentation of assigned colleagues during onboarding, provide feedback to new colleague and Program Leadership, and collaborate with National Director of Education to ensure smooth transition for colleagues/Leaders to their new roles.
  • Reports deficiencies to appropriate staff for corrections and follows up to ensure completion.
  • Assists in development and delivery of educational content throughout Agency to ensure that plans of care are updated to reflect new orders, interventions, and instructions.
  • Collaborates with Program leadership to identify need for additional education to support quality onboarding experience for new colleagues/Leaders.
  • Assesses documentation to ensure the quality of care and services provided are reflected, and identifies educational opportunities to improve documentation to ensures the quality of care and services provided are reflected.
  • Coordinates with agency leaders and colleagues to determine educational needs, and develop and manage delivery of education to support improvement.
  • Maintains proficiency in home health provision of care and treatments including, but not limited to ability to
  • • Explain home health services, Medicare benefits, homebound criteria, skilled need
  • • Explain OASIS and other home health documentation requirements
  • • Explain HCHB and other technology platforms to support home health care delivery, documentation and outcomes
  • • Educate patients and families on safety and infection control in the home, care of the patient, disease process, symptom control, wound care, IV therapy
  • • Educate on home health plan of care development in conjunction with ordering Physician or NPP
  • • Educate on intervention of other IDT members and effectively communicating patient and family needs with IDT
  • • Record observations, treatments, and other pertinent information
  • • Communicate effectively with IDT, Medical Director and Attending Physician, e.g. SBAR reporting, measures to alleviate symptoms and toward prevention of unnecessary emergency room utilization or acute care hospitalization
  • • Educate colleagues on effective Case Management and autonomy in home health
  • • Educate on home health quality measures and patient satisfaction measures
  • • Performs other duties as assigned.

Education and/or Experience

  • Registered Nurse
  • Minimum of three years in Healthcare required, preferably in Home Health or Hospice setting. Active license in the state of employment. Knowledge and experience in quality improvement and utilization review. Home Health OASIS preferred.
  • A valid driver’s license and auto liability insurance. Current Registered Nurse State Licensure as required.
