Community Facilitator III KLEE(JUN,P)
General Description:
We are seeking an energetic, patient, thoughtful, and outgoing person with experience to support & assist a (eg young man, young lady) with daily living skills in (his/her) home and (his/her) surrounding community. You will teach all community safety & awareness, social interactions & self-help skills. The person being supported enjoys (eg watching tv, hiking, special Olympics)
Essential Responsibilities:
- Provide one-to-one support to a individual in (his or her) home & community whom has a diagnosis of Autism or other developmental disabilities & has limited safety awareness.
- Implement the individuals Support Plan as developed
- Provide the necessary accommodations to support an individual with various challenges (behavioral, physical, communication, etc.) within their home and community
- Support the individual to establish a daily/weekly schedule of activities.
- Act as a mentor and role model for socially acceptable and culturally valued ways to interact and support each other to be successful
- Participate hands-on in all activities
- Assist and support the person to maintain their own personal safety and well-being
- Provide information to family members about the unique needs of the individual to assist in the building of a more cohesive family unit.
- Comply with all policies and procedures of the organization and public locations of activities
- Other responsibilities as assigned.
Qualifications: - Experience supporting individuals with Autism and or developmental disabilities preferred not required.
- Energetic, outgoing, patient, and respectful
- Have a high school diploma or GED
- A valid drivers license, reliable auto, current auto insurance, & smart phone are required
- Ability to handle physical aspects of job
- Ability to interact effectively at all levels and across diverse cultures
- Comply with all policies and procedures of the organization and public locations of activities
