JOB SUMMARY: Assists in providing the fundamental aspects of programs or cases and assists in providing client services, including support for families, under close and regular supervision.

ESSENTIAL FUNCTIONS:

  1. Assists with conducting and completing SSI/SSDI applications for clients.
  2. Advocates for clients and provides support for appealed cases
  3. Provides information and referral to clients over the phone or in person
  4. Provides case management assistance, i.e. preparation/maintenance of client files, mailings, client tracking, and technical and administrative assistance.
  5. Assists clients in identifying benefits, social and community services and helps clients obtain them.
  6. Assists with transportation needs
  7. Provides emotional support to clients.
  8. Assists or organize/lead client activities
  9. Assists with crisis intervention
  10. Confers with medical/other personnel on client behalf to complete SSI/SSDI application
  11. Coordinates and presents throughout the Mercer County services available for eligible clients

KNOWLEDGE/SKILLS/ABILITIES:

  • Knowledge and understanding of case management
  • Basic computer and software skills and experience (Microsoft Office)
  • Complete work with accuracy and meet deadlines
  • Communicate effectively, both verbally and in writing; understand and follow directions
  • Complete SSI applications for the target number of clients needed to be served
  • Provide a high level of customer service
  • Establish and maintain effective working relationships with clients, families, caregivers, service providers and staff
  • Knowledge of community resources is highly desirable

SUPERVISORY REQUIREMENTS: N/A

MINIMUM QUALIFICATIONS: Bachelor’s Degree in Social Work or Psychology; 4 years of related case management experience (paid-unpaid).

LICENSES/CERTIFICATIONS NEEDED: Valid drivers license

Bilingual in English / Spanish preferred.

Experience with the homeless population, with a history of mental health and/or substance use a plus.
