RESPONSIBILITIES
- Coordinate with patients, family, healthcare providers to ensure patient can thrive in home setting
- Coordinate care of patient's with team within scope of Medicare Guidelines
- Teach and assist patients on becoming independent in managing their health at home
REQUIREMENTS
- Licensed Practicle Nursing License in the state of Illinois
- 1 year experience of acute care
- Current CPR card
- Meets the requirements of federal and or state criminal and abuse background check
- Must have reliable transportation
