Functional Pathways is a national
contract therapy provider with over twenty-three years of experience serving
senior living communities. As a therapist-owned, medium size organization,
we can uniquely partner with organizations a better way, as
we have the flexibility to break the cookie cutter model offered by other
providers and exceed our clients’ expectations.

From our experience, we know our industry is constantly evolving,
and we have spent the last seven years developing our proprietary software systems
and customized clinical programs that have positioned us to help clients
navigate the new Patient Driven Payment Model (PDPM). As we transition into the
new reimbursement model, we believe our continued focus on Outcomes, Innovation, and Relationships are key to continuing our
delivery of therapy a better way.
