Public Health Nursing (PHN) includes, but is not limited to, providing leadership within systems, communities and individuals in order to achieve optimum health. Primarily PHNs provide case management to individuals with communicable diseases, including Tuberculosis control for the city of Cambridge and CHA TB Clinic in collaboration with the Massachusetts Department of Public Health. This work is done in a variety of setting within CHA and in the community.
Job Purpose: HEALTH PROMOTION Provides broad array of health promotion and disease prevention services_within the Cambridge community
ASSESSMENT AND PLANNING Identifies and develops relationships with key community members and groups interested in promoting wellness and public health concerns, assesses needs and plans health promotion interventions; Recognizes need for and utilizes data to assist in assessment. Seeks appropriate guidance and assistance from supervisor for collecting, accessing and using information.
INTERVENTION, ASSURANCE AND EVALUATION Initiates and participates in community health promotion programs and events which are coordinated, where possible, with other available programs and personnel, Actively participates in ongoing evaluation and improvement efforts for all aspects of health promotion activities, makes home/site visits as necessary. Performs other PHN departmental or community health promotion duties as need emerges.
POLICY DEVELOPMENT Actively contributes to departmental discussion of deployment of existing PHN resources, Provides PHN expertise and seeks opportunities to participate with other health providers and agencies in developing health promotion programs within the community, Acts as advocate for target populations not able or willing to bring their health promotion needs to local policy makers, Participates in efforts to integrate department and health promotion agenda for the population within CHA.
Communicable Disease Prevention and Control Investigates communicable diseases reportable to the City of Cambridge Health Department ASSESSMENT AND PLANNING Identifies communicable disease risks and participates in designing appropriate and effective prevention and intervention strategies, Determines health risk to individual, contacts and the publics' health by thorough nursing and epidemiological assessment. Consults with medical, public health, and city and state officials as necessary.
Qualifications/Requirements: Three years of public health or community-based nursing required. Experience with infectious disease and TB required. PHYSICAL: Ability to move easily around the city, including visits to homes and other settings which may not be easily accessible is required. Able to communicate with a variety of groups and individuals. MENTAL: Ability to organize and prioritize efforts under time constraints and competing demands required. Ability to integrate information from different disciplines required. BSN required
