Job Summary

An Admission RN is responsible for planning, coordinating, providing and documenting skilled care for assigned patients from day of admission until SOC report is given to the RN Case Manager. This position is Part-time.

As needed, this position may assume job responsibilities of an RN Case Manager. Utilizing physician's orders and professional skills, develops and implements a Hospice care plan that meets the patient's specific need and is in compliance with Federal and State regulations, reimbursement guidelines as well as agency policies and procedures. All skilled services must be provided under a physician's order and the Registered Nurse must have demonstrated professional competency prior to performing any skills. All care must be provided in compliance with Federal and State regulations, reimbursement guidelines, scope of practice, and agency policies and procedures.

Performs and/or review admission assessment within required time frame as directed by physician order including completion of admission documentation. Make home visits to admit patient. Develops and initiates a written plan of care in collaboration with physician and IDT members for each patient based on assessment and documentation. Prepares and submits timely and accurate admission documentation, clinical and progress notes, and other agency documentation per agency policy. Coordinates and oversees all services related to care and treatment of patient until the SOC report is given to the RN Case Manager and all disciplines. Informs the physician and other personnel of changes to the patient's condition and needs. Participates in in-service programs, team meetings, QAPI and one-on-one monthly meetings. Communicates clinical information to the staff and IDT members regarding new admissions. Maintains productivity standards per policy. Oversees and teaches other nursing personnel as directed by Clinical Manager. Participates in on-call as scheduled. Notifies the physician and Clinical Manager of admission status within 24 hours. Performs other duties and assists with other projects as assigned. Adheres to HIPAA standards and maintains confidentiality of Heritage proprietary information. Actively contribute to the general well-being of our working environment with a positive and constructive approach to our customers, fellow employees and tasks at hand. Acknowledges, understands and complies with the Compliance Program.

Qualifications

Graduate of an accredited school of professional nursing. Experience as a RN commensurate with one of the following: One year Medicare Home Care experience in the last two years. One-year hospice nursing experience in the last two years. Less than one-year experience may be considered on an individual basis with approval from the Executive Director.

Current state licensure as a Registered Nurse. Current CPR. Current Health Certification. Must be organized, with the ability to exercise independent, sound judgment. Ability to travel in service area and participate in on call. Valid driver's license and current auto insurance.

Must be competent with computers and able to learn medical software quickly.
