Job Description

Provides clinical services that facilitate the assessment process in a timely fashion as outlined by DHH, CARF, Federal and State Regulation Standards. Rotates on-call duties as assigned.

  1. Responds to inquiry calls and entry of information into Salesforce accurately. Schedules admissions and assessments in a timely manner, and documents follow-up calls made regarding pending assessments and inquiries.
  2. Conducts assessments at facility or other locations, completes Intake Assessment and Initial Triage thoroughly.
  3. Refers clients who are not appropriate for facility programs to appropriate resources.

Coordinates Admissions process for clients admitted to facility programs

  1. Completes Admissions Forms/Admit Note accurately.
  2. Pre-certifies admissions and documents pertinent UR information.
  3. Delivers client services in a respectful manner which promotes dignity and client satisfaction.

Demonstrates ability to assess and interpret age/disability/culturally sensitive specific information.                                                                    

  1. Demonstrates knowledge of growth and development.
  2. Communicates in age/disability/culturally sensitive manner to clients and clients' families.
  3. Identifies safety needs including ability to clinically detect abuse and neglect signs.
  4. Identifies special mobility needs.
  5. Identifies psychosocial needs.
  6. Identifies special equipment needs.
  7. Assesses and Interprets age/disability needs to assess level of care needed and make appropriate referrals based on same.
  8. Involves families or significant others in decision making related to treatment as appropriate.
  9. Passes age/disability/cultural sensitivity basic skills test, probationary and annual monitor.

 Provides Afterhours on-Call services for AAC.

  1. Responds to inquiry calls and completes entry of information into Salesforce accurately.
  2. Schedules admissions and assessments in a timely manner, and documents follow-up calls made regarding pending assessments and inquiries.  
  3. Refers clients who are not appropriate for facility programs to appropriate resources. 

Attends Admissions Departmental Meetings and monthly All Staff Meetings

Performs other duties as required.
