Job Description

Behavioral Health Services Care Coordinator needed for a full-time contract opportunity with Yoh's client located in Campbell, CA! See below for details regarding this exciting new opportunity!

Contract Duration: 3+ Months Full-Time

Top Skills You Should Possess:

  • Bachelor's Degree in Health Sciences or related majors such as Psychology, Sociology, Social Work, etc. and/or equivalent experience and coursework
  • Experience working in social services settings
  • Knowledge of Medicare/Medi-Cal benefits and medical terminology
  • Great customer Service Experience; and adapt to a fast-paced changing environment

What You'll Be Doing:

  • Working closely with case management and social work team to refer members to contracted agencies and counselors in the community
  • Members will be screened by staff and determined to have moderate mental health needs
  • Develop a network of community based organizations and providers
  • Reach out to members to complete Health Risk Assessments and work with staff to determine members' level of independence
  • Coordinate with care teams to implement members' individualized care plans and oversee correspondence to care plans
  • Support successful transition of care to members by facilitating services for medical appointments, pharmacy assistance, and utilization review-ensure proper follow up with members to obtain behavioral healthcare
  • Process authorization requests and data entry into the software application system
  • Produce and distribute various reports for metrics, utilization, and stakeholder reports
  • Working with the community to develop professional relationships and networks in order to refer members to the appropriate resources
  • Perform other duties as required or assigned


  • Bachelor's Degree in Health Sciences or related majors such as Psychology, Sociology, Social Work, etc. and/or equivalent experience and coursework
  • Three years of relevant experience in a healthcare or community setting
  • Proficient in adapting to changing situations and alternating focus and tasks to support department operations
  • Ability to work within a team structure
  • Knowledge on how to operate computer software: Microsoft Office and specific case management programs
  • Ability to communicate to members, staff, and managers with professionalism and directness.

Must be Authorized to work in the US without Sponsorship
