Case Manager - Young Adult Services Position Location:

Located at our new state-of-the-art Windham Regional Health and Wellness Center at 140 North Frontage Road, Mansfield, CT 

Position Description:

Work as part of a multidisciplinary team in our Young Adult Services (YAS) Program located in Mansfield, CT. Provide case management, housing services, individual support and group counseling for young adults, ages 18-25. If you have knowledge of Pervasive Developmental Disorder and experience with mental health, substance abuse, community support services for young adults and an understanding of the developmental issues of this population, please consider becoming part of our team. YAS provides a diverse array of services including clinical, medication management, case management, vocational, recreational, transitional living and housing services.

Schedule:

Monday – Friday, 12:30 p.m. – 9:00 p.m.  A flexible schedule is required and may include days, evenings, weekends and holidays.

Minimum Requirements:

Bachelors Degree in relevant human service field required or Associates Degree in human services field, plus two years relevant experience.

Salary Range:

$14.62 – $16.54 per hour depending on experience
