Case Manager


Dayville, CT

(Position is located 30 minutes from Providence, RI -- Worcester, MA -- New London, CT -- Willimantic, CT)


Full-time position in Dayville working with adults with serious and prolonged mental illness. Play a critical role in functional and resource assessment, creating service plans, and assisting clients in skill development. Work in collaboration with other providers within the mental health and social service systems. Assess client needs, provide counseling, assist clients w/developing & managing community service plans. Use of own car required.

Schedule: Monday through Friday, 9:00 a.m. to 5:30 p.m. including one evening per week until 7:00 p.m.

Minimum Requirements: Bachelor’s Degree in relevant human services field or Associate’s Degree in relevant human services field plus two years of relevant experience.

Salary Range: $14.62 - $16.54 per hour depending on experience
