Job Description

Provides Case Management services to adults with mental illness.

  1. Serves as liaison with other agencies (i.e., Probate Office, state institutions, local hospitals, DHR, etc.) in identification and coordination of services for SMI referrals.

  2. Accomplishes initial contact with clients within one week of referral.

  3. Develops and implements treatment plans, which are individualized and directed toward clients' service needs.

  4. Provides direct case management services including service linkage, crisis intervention, service arrangement, advocacy, needs assessment, transportation and transportation management.

  5. Accomplishes regular progress assessment, service evaluation, and treatment plans in accordance with DMH Standards.

  6. Completes all statistical, administrative and clinical documentation within one working day of all pertinent clinical activities.

  7. Submits documentation which is concise, accurate, and in accordance with Center policies and DMH Standards.

  8. Exercises proper judgment as to when and where clients are referred.

  9. Maintains and provides accurate consultation to community agencies.

Provides crisis intervention as needed and when needed including working a variable work schedule, after normal business hours, and on weekends if necessary. 

  1. Responds to emergencies as needed.  Assure clients' safety and welfare through adherence to Emergency and Safety Policies and Procedures.

  2. Ensures the safety and well-being of clients while transporting to Center and other appointments. 

Maintains and provides liaison and education for the Center, other community agencies, and the general public. 

  1. Presentations are presented in an effective and professional manner.

  2. Presentations are accurately targeted toward audience.

  3. Effectively serves as a positive advocate for Mental Health Services in the area. 

Maintains and improves professional skills. 

  1. Attends and actively participates in professional workshops, in-service training, and Quality Assurance review.

  2. Reads current professional periodicals.

  3. Effectively utilizes people on staff for consultation. 

Works effectively with support personnel. 

  1. Provides accurate, timely, and precise information to support personnel.

  2. Maintains effective working relationships with support personnel. 

Maintains productivity requirement of an average of 5 hours per day.

 

Job Requirements

Minimum Qualifications: Bachelor’s Degree, preferably in a Social Science field, a valid Alabama Drivers License, maintenance of a good driving record, and post-hire completion of Department of Mental Health approved Case Management Training.

            Physical and Mental Requirements:

  1. While performing the duties of this job, the employee will be required to communicate with peers, clients and/or vendors.

  2. Performs duties that require the employee to stand and walk for extended periods.

  3. Requires ability to operate a vehicle and make sound judgement while driving

  4. May sit or remain stationary for periods.

  5. While performing the duties of this job, the employee is regularly required to, stand, sit; balance, walk, talk, hear, and use hands and fingers to operate a computer and telephone and computer equipment

  6. Specific vision abilities required by this job include close vision requirements due to computer work and vision requirements for driving a vehicle

  7. Must be able to climb stairs, access and operate elevators, and access patients' homes, which may not have conventional access accommodations. Must be able to maneuver all types of terrain.
