Effectively utilizes hospital resources in the development of care and discharge planning.
Performs prospective, admissions, observation and concurrent review according to standard policies and procedure.
Reviews and analyze processes designed to ensure appropriate utilization of medication, blood products, operative and invasive procedures, and use of resource intensive diagnostic testing and treatment.
Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
Assists with LTC, SNF, Rehabilitation, Behavioral/Mental Health, Home Care, DME, transportation and other placement planning needs.
Assist wit interactions with CPS, APS, Adoptions and end of life planning and completion of Advance Directives.
Is a liaison with third-party payers and government programs to access special services for patients.
Minimum Education: Graduate of a School of Nursing Program.
Minimum Experience: At least 3 years of Utilization and case management experience strongly preferred. Ability to prepare reports and formal presentations preferred.
Certification/Licensure: Current New Mexico or compact RN licensure required. Certification by the National Association of Healthcare Professionals or InterQual is desirable. Certification as a CCM or ACM desired.
