Case Manager - Social Worker

This position is community-based. The Social Work Case Manager is a behavioral health specialist who provides case management services for the chronically ill or at-risk population with an overall objective to help member’s live fuller and more meaningful lives. The Social Work Case Manager will assist in managing members with behavioral health, substance abuse, and/or psychosocial conditions/issues. The Social Work Case Manager also consults with all members on the team on behavioral health, substance abuse, and/or psychosocial issues. As a behavioral health specialist, the Social Work Case Manager facilitates member adoption of strategies to promote physician recommended behavior changes. Social Work Case Managers will help members improve health outcomes and provide feedback to members of the medical care team.


  • Perform member outreach – telephonically and in-person, individual needs assessment, education on early warning signs of disease and disease prevention, and identifying customers in potential need of care. Support services as well as social service supports from other organizations (community based and otherwise).
  • Provide ongoing support and education to patient and family members including regular face to face meetings at the Neighborhood Care and telephonic contact with patient and/or family to assess health status and assure treatment compliance.
  • Directs activities relating to complex social service coordination, including locating and referring patients to community resources and support while working collaboratively with providers to develop creative, cost effective continuing care plans; identifying alternate levels of care as needed and works with interdisciplinary team to achieve improved health outcomes.
  • Support activities of other Neighborhood Care team members.
  • Participate as part of the Quality and Customer Experience team by providing feedback and ideas in order to continually improve clinical metrics and the customer experience.
  • Participate in all other projects relating to community clinical programming as identified by Manager, Community Care Solutions, Manager, Program Development, AVP Community Care Strategy and Medical Management Leaders.
  • Provide general customer-level support for the EH’s offices, which include education and consultation relating to topics on wellness, health screening and simple disease management.

  • Master’s degree in social work with LMSW
  • Fluency in Spanish (reading/writing) strongly preferred
  • Minimum 3-5 years of direct social work experience. Experience in a managed care setting required
  • Working knowledge of community health and support services required
  • Discharge planning or case management experience required
