LPN

SUMMARY 

The Licensed Professional Nurse (LPN) – Senior Services practices within the scope of the Illinois Nurse Practice Act to ensure the delivery of comprehensive, high quality, individualized nursing care, based upon knowledge and experience the nursing process and leadership skills. Coordinates family involvement in the treatment, continuing care of the patient, and ensures completion of patient care tasks by appropriate caregivers.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Attends report, receives assignments, and provides nursing care as assigned; Consults with and keeps supervisor informed of activities, needs, and problems with regard to assigned patients.

2. Receives and transcribes orders. Initiates and keeps care plans up-to-date.

3. Provides direct patient care including:  assisting CNAs when needed, answering call lights, checking charts, administering medications; performing treatments, repositioning patients, giving range of motion exercises, etc.; taking vital signs; changing dressings; inserting and checking catheters, measuring output; reinserting gastric tubes; irrigating colostomies; giving enemas; administering insulin; setting up oxygen and checking oxygen delivery; and checking bed rails and restraints, and assisting with feeding.

4. Draws blood; inserts nasogastric tubes (including radiopague and stylet); hangs IV solutions; attaches IV piggy back solutions after training and competency testing. 

5. Observes patients for color and condition of skin, response to medications, treatments, diet, symptoms of infection or communicable diseases, unusual symptoms, adverse reactions, emesis, drainage, and discharges, bleeding, etc.  Observes patients psycho-social interaction.

6. Performs charting duties, including medications and intake, and reports anything significant or abnormal to supervisor. Maintains required records, reports, and statistics.

7. Maintains patient rights and confidentiality of patient information.

8. Checks stock of supplies and equipment, and reports needs to immediate supervisor. Checks supplies and equipment for proper functioning, and reports defects or suspicions to appropriate person(s).

9. Performs clerical duties, answers telephone, and writes charges. Maintains required records, reports and statistics.

QUALIFICATIONS

Education and/or Experience                             
Graduate of an accredited nursing program required. 
Experience in a long term care or acute hospital setting preferred.

Computer Skills                             

Certificates, Licenses, Registrations 
Licensed Practical Nurse with current Illinois License and Basic Life Support (BLS) certification required.

Patient Handling Category    
High Level: Normal routine includes but are not limited to dependent patient transferring tasks, lifting tasks, repositioning, tasks, bathing patients in bed, making occupied beds, dressing patients, turning patients in bed, and tasks with long durations. When a lift or carrying task involves more than 35 lbs. of the patient’s weight, or is unpredictable and could exceed 35 pounds, then assistive devices should be used.

 Medium Level: Normal routine includes partial patient assists that require no more help than stand-by, cueing, or coaxing, or caregiver is required to lift no more than 35 lbs. of a patient’s weight.

 Low Level: Duties performed routinely do not include patient transferring tasks or patients are Independent - that is: patients perform tasks safely, without staff assistance or without assistive devices.

Object Handling Categories

Work requires the ability to lift/carry objects routinely as follows:
Heavy Lifting: Objects weighing 35-50 pounds lifted on a regular basis. Mechanical lifting devices (carts, dollies, pallet jacks, forklift trucks, etc.) or team lifts should be utilized.
Medium Lifting: Objects weighing 15-35 pounds lifted on a regular basis.
Light Lifting: No lifting of objects weighing more than 15 pounds on a regular basis.


Other Physical Demands (check all that apply)
• Work requires the ability to stand up to two or more hours at a time
• Work requires the ability to stoop, bend, reach and grab with arms and hands, manual dexterity
• Work requires sufficient auditory and visual acuity to interact with and/or care for others

OSHA Exposure Category  
                           
Category I: Duties performed routinely require exposure to blood, body fluid and tissue.
Category II: Normal routine involves no exposure to blood, body fluid or tissue, but exposure or
potential for exposure may occur.
Category III: Normal routine involves no exposure to blood, body fluid or tissue and as part of their 
employment, incumbents are not called upon to perform or assist in emergency care or first aid.

TB Exposure

• The incumbent works in a patient care area; works in an area where patients enter; works directly with patients; and/or works with specimens that could contain Mycobacterium tuberculosis.
• There may be an occupational risk for exposure to TB and the incumbent is required to take TB control training, PPD skin tests and/or health screenings.
• The incumbent works with patients who have known or suspected TB; enters isolation rooms.
• There is an occupational risk for exposure to TB and the incumbent is required to take Particulate Respirator Fit testing, in addition to TB control training, PPD skin tests and/or health screenings.
• There is no occupational risk for exposure to TB.

CORPORATE COMPLIANCE 

Responsible for remaining up-to-date on all Federal, State and local laws, accreditation standards or regulatory agency requirements, which apply to the assigned area of responsibility and ensures compliance with all such laws and regulations.   

Will comply with all aspects of the Presence Health Corporate Compliance Plan to include the immediate reporting of any known or suspected illegal or unethical behaviors, criminal conduct or patient/employee safety violations or issues.
