Promotion of wellness; coordinated medical appointments with the agency Health Services and remains aware of medical status of individuals. Attends appointments as scheduled and follows through with recommendations. Is aware of the medical status of person being supported at all times. Follows directions of Health Services as directed.
Notifies the director in an accurate and timely manner of unusual incidents, problems, and changes in individual Person Centered Plans/ISP
Ensures the environment remains free of hazards for the individuals we support
Holds fire drills as assigned and records promptly on the appropriate forms
Coordinates, prepares, and reviews the household and maintenance records and ensures they are kept accurate, completed and up to date
Ensures the programmatic and behavioral routines are followed as established; follows established procedures when making changes
Evaluates and appropriately acts in emergency situations, assists as needed. Follows sound directions of supervisor when given.
Complete grocery shopping as necessary (with individual is required if person wishes to… otherwise preferred)
Serves as a partner person for incident management
Complete one overnight check monthly
Leading Others/ Supporting the Direct Supporters
Provides a positive role model
Promotes a positive working relationship with all employees
Displays principles of good management and applies these principles in daily work performance
Supports Direct Supporters in assigned roles
Holds monthly supervision meetings with support staff
Interviews prospective employees.
Coordinates allotted work schedule and assigned duties with supervisor. Works as part of the schedule 32 hours weekly, or as instructed, in the home as direct care.
Participates in the training and evaluation of Direct Support Staff
Fills-in for support staff when necessary
Provides direct support as needed in Emergency Situations
Makes recommendations for hire, supervision, and termination of employees
Provides education/ Directions/ Shadowing to all new employees
Mentors/ Trains all supporters on what works, what does not, important to and important for
Completes the house managers duties checklist of formally completed reports to regional directors each Wednesday
Mentors/ Trains all supporters on the one page description
Document Overtime logs & lateness reports and develops and implements plan of corrections
Completion of supporters evaluations
Serves as an on-call officer; documents time spend performing on-call duties to submit for compensation.
Completes emergency response drills
Provides positive reinforcement for supporters that leads to a higher retention rate
Supporting the agency, Mission & values
Demonstrates effective administration of assigned units; compiles with established administrative policies and procedures
Abides by established policies and procedures of the agency, County and State; offers direction and supervision for compliance for Direct Support Staff
Prepares for, and participates in, all monitoring’s and inspections as directed/requested
Ensures ongoing compliance with the 6400 regulations & provider Qualifications
Ensures Agency Resources are used proficiently by relieving all home related expenses
Participates in required in-service training and professional growth activities
Makes independent decisions regarding operations with her/his authority
Supports community home(s) under their supervision
Provides input to their supervisor to facilitate contacts with County, State, Day Programs, and Community Services
Coordinates and reviews the individual’s records and ensures that they are kept accurate and up to date. Reviews all daily documentation, ensures completion.
Records and reports data to Program Specialist. Follows contracts concerning employee management (tally)
Promotes a positive reputation of PDSI and to people being supported at all times
Attends all individual related trainings/ meetings
Mandatory attendance at monthly house manager meetings and other mandatory meetings
Completes three community home reviews weekly. Takes/ Makes corrective actions
Completes all other assigns tasks as assigned by Person Directed Supports, Inc.
This human services role is a great opportunity for dynamic and compassionate individuals who enjoy helping others. Training is provided but there are a few basic requirements that you must meet to qualify.
Bachelor’s Degree and 2 years of experience working with people with intellectual disabilities.
4 years of experience working with people with intellectual disabilities.
Must be 21 years of age
Must have reliable transportation and a valid driver’s license in good standing
