GENERAL SUMMARY:


Responsible for supporting and working in tandem with the Returning Home Ohio Program Manager.  Perform administrative and client support duties related to the program, streamlining the flow of information to and from clients and staff. Core functions including but not limited to: expenses, internal and external reporting, management  of referrals to the RHO program, data entry and management, responding to client material need requests, providing transportation support to clients, performing assigned interventions to RHO clients under the supervision of the RHO Program Manager, developing and maintaining communication with community partners, participation in case review, providing program coverage in the absence of other team members, and participation in ongoing supervision and training.


 


ESSENTIAL DUTIES AND RESPONSIBILITIES:


 


  1. Work with Returning Home Ohio Manager on expenses for the program through appropriate requests, tracking, and reporting; submit and track check requests for rent, utilities, keep accurate records of all program expenditures.
  2. Assist Returning Home Ohio Program Manager in required internal and external reporting. Maintain data collection in web-based data collection system.  Assist with monthly and quarterly program reports.

    1. Communicate status on above noted reporting with Returning Home Ohio Manager.
    2. Ensure that reporting is completed for review by RHO Program Manager prior to established submission dates.
    3. Work to enhance the effectiveness of tools and processes used for data entry and management.

  1. Assist RHO Program Manager with referrals to the program. 
    1. Provide responses to referrals within appropriate time frame. 
    2. Work with community partners to collect needed verifications to support an initial determination of eligibility.
    3. Track, maintain and follow up on all referrals to program
    4. Assist RHO Program Manager in client selection to the program and with orienting new clients to the RHO Program.

  1. Assist RHO Program Manager with referrals to the program.

  1. Perform assigned client services and interventions to facilitate execution of goal plans as co-authored by the client and RHO Program Manager; focus on securing/maintaining housing, operating from a “housing first” model.
    1. Perform specific interventions that reflect client strengths and needs, note client responsibilities and action steps to help clients in achieving their goals.  May include assisting clients with referral, linkage and follow up to partner agencies and community-based social services and gathering information on potential community resources.
    2. Assist community partners such as landlords, service providers etc. with requests for needed documentation.
    3. Provide budgeting and other planning assistance for the goal of housing attainment and retention
    4. Field requests from clients directly or indirectly for assistance with material needs.
    5. Identify appropriate avenues for addressing the material need and arrange delivery
    6. Coordinate with other Faith Mission Supportive Service programs/staff to assist clients in seeking income, employment, housing and/or other services for clients as appropriate.
    7. Assist RHO Program Manager in identifying housing opportunities for clients.  May include outreach/onboarding of new landlords from the community.
    8. Organize and facilitate monthly program meetings with active RHO clients
    9. Complete meaningful activity reports with active RHO clients
    10. Coordinate and schedule with Returning Home Ohio Program Manager plans for basic transportation of clients including working with available resources and exploring alternatives when a transportation solution is not readily available.

  1. Assist RHO Program Manager in maintaining client files.
    1. Maintain clear and precise data case notes, including careful documentation of all client interactions.
    2. Chart filing and qualitative auditing as instructed by RHO Program Manager

  1. Participate in regular case review meetings.
    1.  Report on client progress, concerns, and continuous improvement for the Returning Home Ohio program. 
    2. Assure documentation of all case review meetings.

  1. Actively participate in regular supervision and training including attending all mandatory CSH trainings and meetings.

  1. Contribute to an atmosphere of safety, sanctuary, dignity, support and resource through adherence to the Faith Mission Code of Ethics and Confidentiality Agreements.

  1. Perform other duties as required.


          EDUCATION:

  • Degree in Social Work or related field or High School Diploma with equivalent experience required; LSW preferred

     

    EXPERIENCE:

  • Prior experience working in a social service setting required

  • Experience working with following or related populations: homelessness, behavioral health, substance abuse/addiction, chronic physical disabilities, HIV/AIDS

  • Experience utilizing the following techniques, interventions and EBP’s: Motivational Interviewing skills, Stages of Change, Harm Reduction.

  • Administrative reporting experience; outlook, Microsoft office, Excel

  • Prior office experience, customer service, property management helpful

     

              REQUIRED KNOWLEDGE, SKILLS & ABILITIES

  • Understanding of and enthusiasm for “housing first” model

  • Dedication to excellent customer service, dignity and respect

  • Excellent computer skills including the use of Microsoft Outlook, Excel and Word

  • Strong oral, written and time management skills

  • Attention to detail

  • Commitment to and aptitude for working in a team environment

  • Ability to lift 25 pounds

     

    OTHER REQUIREMENTS: 

  • Valid Ohio driver’s license and ability to meet LSS insurance standards required

Faith Mission started on February 6, 1966, in response to the City Mission closing its doors. Reverend Sam Goers, Roland Hepner and Arthur Basye of Trinity Lutheran Church were the original board members and volunteer staff. At that time, we were located at 181 East Long Street. The services offered at Faith Mission in 1966 included opening the doors at 5:00 p.m., serving a meal and providing beds for twelve men. In 1998, Faith Mission joined the Lutheran Social Services family.

Today, Faith Mission is a 24-hour emergency shelter, serving up to 199 men and 38 women at two locations: 151 N. 6th St. and 599 E. 8th Ave. Faith Mission is the only organization in Franklin County able to serve three meals a day, every day of the year. Our mission is to continue to create a better world by serving people in need.
