Job Description

Responsible for planning, developing, organizing, implementing and directing resident social service activities  in accordance with current existing federal, state, and local regulations, as well as Five Star established policies and procedures;  to ensure that medically related social services are provided to all residents in Health Care.  

  • Plan, develop, organize, implement, evaluate and direct the social service programs of Health Care.
  • As part of the admission process, meet with the resident and/or family to review policies on advance directives, resident rights and discrimination.
  • Conduct and document a psychosocial history and assessment, including identification of resident problems/needs.
  • Provide for and arrange for social work services as indicated by resident needs.
  • Assist resident with obtaining concrete needs such as clothing.
  • Communicate with resident’s family re: resident needs, transfers, discharge and changes in level of care and financial responsibility.
  • Demonstrates knowledge of and respect for the rights, dignity and individuality of each resident in all interactions. 
  • Reviews and develops a plan of correction for social service deficiencies noted during survey.
  • Acts as backup to admissions coordinator.

Job Requirements

  • At least one year supervised experience in a health care setting working directly with clients/residents.

  • Demonstrated leadership skills applicable to a role in an organization such as a long term care facility. 

  • Working knowledge of and ability to apply professional standards of social work practice in job situations. 

  • Current knowledge of state and federal laws and regulations that apply to the practice of social work in long term care. 

  • Must demonstrate excellent verbal and written communication skills; must be able to communicate with staff on a variety of levels; ability to communicate and work closed with Administrator, DON, Management Team and nursing staff.

Social worker discharge planning preferred.
