United Services, Inc. Job Title: ABI Community Integration Specialist Location: Mansfield Center, CT

Located at our new state-of-the-art Windham Regional Health and Wellness Center at 140 North Frontage Road, Mansfield Center , CT

Description:

Responsible for coordination between the State of Connecticut Department of Mental Health and Addiction Services (DMHAS) and the private sector to facilitate transition and integration of DMHAS consumers with brain injuries into community-based supports and services. Responsible for comprehensive bio-psychosocial assessments to identify appropriate supports and services for individuals referred from hospitals and community providers. Facilitates transitions from hospitals to community based services. Provides consultation and produces written consultation reports. Acts as a liaison to providers and helps to facilitate productive team functioning. Clinical supervision provided by DMHAS Acquired Brain Injury Services. Administrative supervision provided by United Services. Position is located in our Mansfield Center office with one day per week at the DMHAS office in Middletown.

Minimum Requirements:

MSW (CT-LMSW required). Willingness to secure and maintain licensure as a CT LCSW. Three years of experience providing behavioral health services to individuals and families preferred. Demonstrates cultural competency and knowledge of mental health and substance use disorders. Valid driver’s license and reliable insured car required.

Salary Range:
  • LMSW - $41,184 – $46,596
  • LMSW - $43,269 – $48,955
  • CT LCSW $44,096 – $49,891
  • CT LCSW plus Bilingual Spanish Fluency $46,328 – $52,416

Hiring Bonus Available!

$1,000 – CT LMSW

$1,200 – CT LCSW
