The LPN is responsible for the accurate assessment of each patient’s status, problems and needs.The LPN collects and analyzes data from the patient’s medical history, nursing assessments and staff input on an ongoing basis.The LPN provides PRN visits on patients who call the answering service, crisis care visits as assigned and PRN visits as requested by PCC or MD.All care directly provided or coordinated by the LPN is in conformance with federal/state regulations. Differential paid when working. Provides care for patients for the entire County.
PRN visits, crisis care visits, information visits, tuck-ins, coordination of discharge of patient’s from hospitals and skilled nursing facilities for patient’s entering the HOSF program.
Collects and analyzes data from patient’s medical history, nursing assessments, and staff input on an ongoing basis.
Keen clinical observation of patient’s condition, response to treatment/medications and recommend changes to patient’s Plan of Care.
Coordinates care of patients with other HOSF team members through case conferences, consultations with MD’s and community agencies.
Prepares complete and accurate documentation in accordance with all HOSF policies and procedures, Federal/State regulations and Medicare/Medicaid guidelines.
Maintains strict confidentiality and respect for patient/family privacy and possessions.
Practices Universal Precautions and safety at all times.
Teaches patient/family skills needed to maintain patient in comfort and perform personal hygiene activities.
Provides emotional support to patients, family and other caregivers.
Requires a valid, current Florida LPN license and a valid Florida driver’s license.
EXPERIENCE/SPECIFIC SKILLS AND ABILITIES REQUIRED:
Minimum three years clinical experience, preferably in hospice, home health, oncology, acute care or public health.Ability to read, write and speak English.Bi-lingual preferred.Ability to manage time and triage phone calls received from patients, families and physicians.Excellent communication and interpersonal skills.
Ability to lift, push, pull twenty five (25) pounds without difficulty.Good visual and audio acuity.Able to work under varying degrees of stress.
