Community Nursing Specialist
Full-time opening available in Willimantic, CT for a Nursing Professional (RN/LPN) to perform initial health assessments. Develop and coordinate care plans including referring to and coordinating with enrollee’s medical and behavioral health providers. Provide transitional coordination upon discharge from higher levels of care. Provide health education, wellness promotion and prevention initiatives. Provide support and education to clients and families. Provide consultation and training to non-medical staff. Monitor health information tools and reports for treatment, medication alerts and hospital admissions/discharges.
Seeking applicant to staff the Behavioral Health Home program. The Behavioral Health Home (BHH) is intended to integrate behavioral health care with medical care for adults with serious mental health issues who also have serious and chronic health conditions. Through an array of services the goal is to increase access to care, improve the experience of care, and to reduce unnecessary emergency and inpatient care while improving health outcomes.
Monday through Friday, 8:30 a.m. – 5:00 p.m. This position has paid holidays off and no on-call coverage.
Salary Range: $52,000 - $54,000
