Certified Nursing Assistant

SUMMARY 

The Certified Nursing Assistant (CNA) – Senior Services performs various duties under the direction and supervision of a professional licensed nurse to assist the nursing staff in meeting patient/resident treatment and care requirements. All duties are carried out in accordance with the established policies and procedures.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Provides direct patient/resident care as assigned by the licensed nurse and communicates patient’s response.

2. Performs routine procedures e.g. blood glucose monitoring, vital signs, heights, weights, intake and output and the collections of urine, stool and sputum specimens.

3. Documents care per departmental standards in electronic medical record.

4. Assists with patients’ activity and providing a safe environment. 

5. Observes patients/residents while performing delegated tasks and reports significant findings regarding safety, personal needs and changes in patient’s condition to the licensed nurse.

6. Promotes and maintains an atmosphere for patient comfort. Keeps patient/residents’ room clean and orderly. 

7. Provides skin care and is knowledgeable in care needed to maintain or improve skin integrity.

8. Collaborates with the licensed nurse in implementing and evaluating the plan of care and functional status.

9. Assists with admissions, transfers, morgue transport and discharge of patient/resident.

10. Replenishes patient’s supplies as needed.

11. Identifies patient/family education needs and reports them to the licensed nurse.

Certified Nursing Assistant

QUALIFICATIONS 

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Education and/or Experience       
                 
  • Must have completed at least eight years of grade school or provide proof of equivalent knowledge.
  • Must be at least 16 years of age.
  • Previous experience in senior services preferred.

Computer Skills                              
Knowledge of EPIC preferred.

Certificates, Licenses, Registrations 
Certified Nursing Assistant certification required. Basic Life Support (BLS) certification required.

The patient populations cared for by the employee in this job include:

WORKING CONDITIONS

Physical Demands 

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Patient Handling Category    
 High Level: Normal routine includes but are not limited to dependent patient transferring tasks, lifting tasks, repositioning, tasks, bathing patients in bed, making occupied beds, dressing patients, turning patients in bed, and tasks with long durations. When a lift or carrying task involves more than 35 lbs. of the patient’s weight, or is unpredictable and could exceed 35 pounds, then assistive devices should be used.


 Medium Level: Normal routine includes partial patient assists that require no more help than stand-by, cueing, or coaxing, or caregiver is required to lift no more than 35 lbs. of a patient’s weight.


 Low Level: Duties performed routinely do not include patient transferring tasks or patients are Independent - that is: patients perform tasks safely, without staff assistance or without assistive devices.

Object Handling Categories

Work requires the ability to lift/carry objects routinely as follows:
Heavy Lifting: Objects weighing 35-50 pounds lifted on a regular basis. Mechanical lifting devices (carts, dollies, pallet jacks, forklift trucks, etc.) or team lifts should be utilized.
Medium Lifting: Objects weighing 15-35 pounds lifted on a regular basis.
 Light Lifting: No lifting of objects weighing more than 15 pounds on a regular basis.

Other Physical Demands (check all that apply)

  • Work requires the ability to stand up to two or more hours at a time
  • Work requires the ability to stoop, bend, reach and grab with arms and hands, manual dexterity
  • Work requires sufficient auditory and visual acuity to interact with and/or care for others
  • Work requires color vision

OSHA Exposure Category                              
 Category I: Duties performed routinely require exposure to blood, body fluid and tissue.
 Category II: Normal routine involves no exposure to blood, body fluid or tissue, but exposure or
potential for exposure may occur.
 Category III: Normal routine involves no exposure to blood, body fluid or tissue and as part of their 
employment, incumbents are not called upon to perform or assist in emergency care or first aid.


TB Exposure
  •  The incumbent works in a patient care area; works in an area where patients enter; works directly with patients; and/or works with specimens that could contain Mycobacterium tuberculosis.
  • There may be an occupational risk for exposure to TB and the incumbent is required to take TB control training, PPD skin tests and/or health screenings.
  • The incumbent works with patients who have known or suspected TB; enters isolation rooms.
  • There is an occupational risk for exposure to TB and the incumbent is required to take Particulate Respirator Fit testing, in addition to TB control training, PPD skin tests and/or health screenings.
  • There is no occupational risk for exposure to TB.

CORPORATE COMPLIANCE 

Responsible for remaining up-to-date on all Federal, State and local laws, accreditation standards or regulatory agency requirements, which apply to the assigned area of responsibility and ensures compliance with all such laws and regulations.   

Will comply with all aspects of the Presence Health Corporate Compliance Plan to include the immediate reporting of any known or suspected illegal or unethical behaviors, criminal conduct or patient/employee safety violations or issues.

SERVICE PROMISES
Responsible for reading and understanding the Presence Health Service Promises, and committing to modeling and upholding the values of Honesty, Oneness, People, and Excellence.  

Will sign an annual commitment statement and will be held accountable for the Service Promises daily and during the annual performance review.
