Affinity is currently looking for a Population Health RN to work from 8:00AM – 5:00PM, Monday through Friday. This is a Work From Home opportunity for LA/MS/AR residents.

Job Summary: The Population Health RN is responsible for making outreach calls to Vantage Health Plan (VHP) members who have been discharged from the hospital for telephonic medication reconciliation and/or to schedule a Health Risk Assessm ent (HRA) visit with a Nurse Practitioner (NP)/Physician Assistant (PA). The RN will provide VHP members with appropriate health education specific to the member’s individual needs. The RN will also schedule follow-up appointments with the member’s primary care provider (PCP) as well as preventive service appointments as needed.

Essential Duties & Responsibilities:

  • Provides telephonic assessments of member’s needs including medication reconciliation post-discharge for VHP members
  • Provides telephonic patient education regarding follow up care with the PCP and addressing barriers to their plan of care to help decrease re-admission rates
  • Schedules members for Health Risk Assessment visits by the NP/PA
  • Assists with the development, planning, and implementation of the Transitional Care Program within the Population Health Department
  • Retrieves and analyzes data within Vnet, Acuity, Q-NXT, and Centricity on an ongoing basis regarding VHP inpatient admissions and discharges
  • Communicates with the NP/PA and other team members for effective coordination of care
  • Access, collect, and transfer member information from multiple computer data systems
  • Documentation within Excel, Vnet, Q-NXT, and Centricity
  • Assist VHP members with Affinity transportation services as needed
  • Providing ongoing nursing care (assess, plan, intervention, evaluation, referrals) per age specific and developmental needs of the patient
  • Determines priorities of patient care based on essential needs. Anticipates and recognizes changes in patient’s status and need for care
  • Participates in quality assurance activities
  • Integrates health promotion and preventive aspects of care
  • Works collaboratively with other departments and team members within VHP and Affinity Health Group (AHG)
  • Collaborates with the PCP when need arises
  • Reports all information back to the PCP for continuity of care
  • May assist with wellness coupon project, chart reviews, card activation project, or other new projects that arise
  • Other duties as assigned

Marginal Duties:

  • Assists the Population Health Manager and Supervisor as needed with other duties within the role and is flexible to new ideas
  • Able to work independently and interdepartmentally to obtain departmental and organizational goals

Knowledge, Skills, and Abilities: 

  • Current licensure as a Registered Nurse (RN) granted by the State of Louisiana
  • Must have an active RN License to practice in the state of Louisiana
  • 2+ years of clinical experience in medical-surgical or other hospital specialty unit
  • Case management skills preferred
  • Exceptional interpersonal, verbal, and written communication skills with concise, clear documentation skills
  • Good judgement and critical-thinking skills
  • Ability to work with limited supervision and to make decisions based on established policies and procedures
  • Must have a compassionate and caring manner with patients to form an instant rapport with the caller
  • Excellent phone etiquette and ability articulate and communicate well to individuals of all educational levels
  • Excellent negotiation skills
  • Functions well in a high-stress environment and under time pressure
  • Open and adaptable to change


  • Current licensure as a Registered Nurse (RN) granted by the State of Louisiana
  • Current basic life support (BLS) certification 


  • 2+ years of clinical experience in medical-surgical or other hospital specialty unit
  • Case management skills preferred

Supervisory Responsibilities:

  • None

Supervision Received: 

  • Works under general supervision of the Population Health Manager and Supervisor

Working Conditions:

  • Monday-Friday 8:00 AM- 5:00 PM
  • Overtime may be occasionally required
  • Travel may be required and is reimbursed according to company policy
