Our Locations and Service Areas
We proudly serve patients and families in private homes, assisted living facilities, skilled nursing facilities, hospitals, and our care centers located in Wilmington, Bolivia, and Whiteville, North Carolina. Our service area includes Bladen, Brunswick, Columbus, New Hanover, Onslow and Pender Counties in North Carolina; and Horry, Georgetown and Marion counties in South Carolina. Our corporate office is in the coastal community of Wilmington, NC.

We are currently seeking a Certified Nursing Assistant – Home Care to join our group of dedicated and experienced professionals.
This is an outstanding opportunity to join the largest and most experienced hospice in the area. Lower Cape Fear Hospice is certified by Medicare and Medicaid and approved by the Accreditation Commission for Health Care, Inc. Each day, more than 600 patients and their families receive care from Lower Cape Fear Hospice in our hospice and palliative programs.
Knowledge, Skills and Abilities:
Competently performs a variety of basic client care activities under the supervision of the RN to meet the personal needs and comfort of clients. CNA supervision must include following the Plan of Care, task competency, compliance with infection control and reporting changes in client condition. In addition, the CNA must have a competency evaluation by an RN every 90 days to evaluate services of CNA.
Appropriately documents and communicates all required information in accordance with agency policies and procedures.
Successful completion of a Dementia training course prior to providing services to Dementia clients.
Successful completion of orientation in hospice prior to providing services to hospice patients
Organizes and performs work effectively and efficiently.
Maintains and adjusts schedule to enhance team performance.
Demonstrates positive interpersonal relations in dealing with all members of the team.
Maintains and promotes customer satisfaction.
Effectively demonstrates the mission, vision, and values of the agency.
High school diploma or equivalent required.
Completion of a CNA course required.
Must attend 12 hours of in-service training supervised by a Registered Nurse on a rolling 12-month basis. Competency of the CNA is assessed in the home every 90 days. The CNA does not need to be present unless there is an area of concern. Otherwise, an onsite evaluation occurs once a year.
Has not had a lapse of 24 consecutive months where the aide has not provided services for compensation.
Current certification with State Board of Nursing, or current registration with Department of Facilities Service in the State of North Carolina/South Carolina required.
Completion of a course on caring for a patient with Dementia prior to providing services to Dementia patients required.
Completion of Hospice orientation prior to providing services to hospice patients required.
Minimum of two years’ experience and at least one year in home care or by other individuals under general supervision of a Registered Nurse required.
