 Registered Nurse - RN

Companion Hospice has openings for Full-Time and Per Diem Registered Nurses.  A Home Care Registered Nurse provides skilled nursing care including skilled assessment, planning, care, instruction in the home under a plan of treatment, and orchestrates the assignment of ancillary services to provide quality care for the assigned case load.

Registered Nurse - RN

General Duties:

  • Directs and evaluates patient care; coordinates the Plan of Care Directs; monitors and directs patient care to ensure that treatment plans and instructions are effective, current and coordinated with appropriate resources
  • Coordinates care of the patient with other care providers, including attending care plan meetings, and using community resources as appropriate.
  • Communicates with other members of the team regarding patient care needs and any change in condition
  • Supervises and documents supervision of Aides by ensuring the Aide is providing care as directed and documentation of care matches the written Plan of Care, completes documentation of supervision, and ensures the Aide Plan of Care is current, revised as necessary and is individualized.
  • Plans for and provides patient care in accordance with the Nurse Practice Act by educating the family and patient in safe and effective interventions and disease process, provides care according to physician orders and standard nursing practice, obtains new orders as necessary and informs the team and physician of any change in condition, plans for interventions to control symptoms.
  • Demonstrates commitment to professional growth by attending appropriate meetings on a regular basis and consistently demonstrates self-direction in improving skills.
  • Documents patient assessment and on-going care concisely according to policy and nursing standards; documents care clearly using accepted standards of documentation and abbreviations, completes visit notes, physician orders, care plans, medications profiles and other documents according to state regulations and policy.
  • Meets expected productivity guidelines of 5-6 patient visits per day; develops and maintains visit schedules with patients & caregivers; carries a patient case load as directed.
  • Educates the patient and family in home care philosophy and procedures as appropriate; prepares the family for discharge; provides education on emergency procedures and home safety; educates the patient and family in medication use and side effects.

Registered Nurse - RN 


  • Preferably a BSN degree from a nursing program accredited by the National League of Nursing
  • At least 1 year of either of the following:
    • 1 year of varied work experience as a professional nurse
    • 1 year within a home health setting within the last three years
  • Possession of a current license to practice as a registered professional nurse in the State of California.
  • Demonstrates the ability to assess and respond to the needs of patients and families in varied settings.
  • Skilled in nursing practices
  • Able to cope with family emotional stress
  • Tolerate individual lifestyles.
  • Demonstrated skill in administrative performance
  • Demonstrated ability to execute a supportive, sensitive approach to staff and patient/family.
