SUMMARY OF RESPONSIBILITY

The Licensed Practical Nurse is responsible for providing nursing services to patients in the home or community setting under the direct supervision of a Registered Nurse. All functions shall be performed in accordance with the established policies and regulations and standards of care for a Licensed Practical Nurse.

Essential Functions

1. Provide professional nursing care as defined in the nurse practice act and under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per the Company’s guidelines.

2. Provide skilled nursing visits for patients as directed by the RN Case Manager. Follows a care plan established by the Registered Nurse.

3. Work closely with other members of the interdisciplinary team to enhance care and support for the patient and family.

4. Scope of clinical practice includes personal care services, monitoring and reporting findings, medication administration and monitoring, emergency intervention, and nursing serves within the professional scope and as delegated by the RN Case Manager.

5. Demonstrate knowledge of safety/infection control practices by compliance with policies and procedures.

6. Ensure quality and safe delivery of hospice services to patients, families, and caregivers.

7. Ensure that equipment, supplies, and other necessary items and services are available to the patient.

Communication

1. Initiates regular communication with attending physicians and hospice team members regarding the patient’s needs and reports changes in patient condition.

2. Communicates with community health related persons to coordinate the care plan.

3. Instructs the patient and family/caregiver of self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.

4. Works in cooperation with the family/caregiver and interdisciplinary group to meet the emotional needs of the patient and family/caregiver.

5. Attends interdisciplinary group meetings.

Additional Duties

1. Participates in on-call duties as defined by the on-call policy.

2. Ensures that arrangements for equipment and other necessary items and services are available.

3. Monitors all documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness via electronic record so that problems, actions, and goals are accurately and clearly stated and changes are reflected as they occur, including new information obtained during IDG meetings.

4. Service is expected to be documented during, or by close of business day in order to maintain an accurate medical record.

5. Participates in all mandatory staff in-services, meetings, and/or education.

6. Fulfills the obligation of requested and/or accepted case assignments.
