Case Manager (PLL Program)

The Parenting with Love and Limits (PLL) program provides community based services to youth and caregivers who are at risk of out of home placement or who are currently placed in a residential setting and expected to return home within the next six months. This program is an evidenced based model focusing on youth ages 10-18 years old with extreme emotional or behavioral problems including running away, extreme disrespect, chronic truancy, depression, and drug and alcohol abuse. The average service period is 7-9 months depending on when a youth returns home. Once a youth returns to the community, the program commits to staying in place for an additional 90 days post discharge. This model requires each staff member to complete a full five day training in the model. The model consists of an in office parent and youth group in addition to in home coaching sessions. The youth are also provided with case management services that focus on school and extracurricular activities in the community.
The case manager is responsible for working with the identified caregiver on things such as community referrals, public assistance and housing. This worker is also responsible for facilitating the youth group sessions as part of the PLL model. This worker will have communication with the school the youth is attending as well as increasing the youths engagement in extracurricular activities. The case manager is also responsible for attending all scheduled court dates for their assigned caseload. Assistance with transportation to client appointments or meetings in the community is also an expectation of this position. This worker would be expected to be available for trainings and occasional meetings that occur in the other NAFI NY offices. All case contacts are expected to be documented in an electronic health record and communication to our contract monitors is expected on a regular basis including participation in monthly case conferences. This candidate is required to have their own vehicle and will be reimbursed for travel associated with work responsibilities.
