Job Summary: The Pharmacist is licensed by the California State Board of Pharmacy. Under the general supervision of the Director of Pharmacy and/or the Senior Pharmacist(s), the Pharmacist is responsible for the daily organization and supervision of all drug distribution activities in the hospital, including involvement in clinical endeavors.
Reviews and validates medication orders for safe medication use. Acts as a drug information resource to patients, medical staff, nursing staff and ancillary department personnel. Compounds and dispenses prescribed medications and other pharmaceuticals for patient care by performing the related duties.
Performs all aspects of patient care in an environment that optimizes patient safety and
reduces the likelihood of medical/health care errors
Understands and is able to use the hospital EMR system to access relevant information from the patient’s medical record (allergies, labs, notes…)
Reviews and validate medication orders
Manage work queue, and process orders in a timely manner
Able to place orders using CPOE Order Sets
Able to place non-medication orders (i.e. labs) in the EMR system
Monitor medications orders for drug/drug interactions, duplication of orders, potential adverse effects, and allergic reactions
Determine compatibility or rationality of drug therapy
Follow non-formulary medication request procedures for safe medication use
Able to identify clinical interventions, and make recommendations to the physician for therapeutic changes
Demonstrates ability to apply mathematical operations to such tasks as dosage calculations, formulation of pharmaceutical preparations and data analysis
Pending orders, or issues, are resolved in a timely manner for safe patient care
Effectively doses and monitors pharmacist managed dosing regimens
Reviews and monitors drug therapy that require dose adjustments
Able to effectively dose and monitor vancomycin and aminoglycosides
Able to effectively perform renal dose adjustments
· Current Pharmacist certification or licensure in the State of California (required)
· One year Pharmacy experience, preferable institutional, or the completion of a residency in institutional pharmacy practice or a Pharm D degree (preferred)
Prior Acute Hospital experience is required.
