Job Title: Physical Therapist Profession: PT Specialty: Home Health Duration: 13 weeks Shift: 8H Days Hours per Shift: 8 hours Experience: Minimum of 2 years License: Required Certifications: BLS Certification Must-Have: - Experience with Oasis (government mandated form) - One year of experience working in PT/OT/Speech - Home setting experience preferred but not required Description: Unit: PT, OT, Speech (home-based care) BLS certification is required. Experience with Cerner EMR is preferred. Rehab aides/OTA/PTA may be available, usually as permanent staff. Every third weekend is a requirement. Minor holidays are required, with major holidays only as needed. Uniform: Business attire with a white lab coat; no jeans permitted. Majority of work hours are Monday through Friday from 8 am to 5 pm. Candidates will be assigned to a geographical region. Floor-specific orientation lasts between 1.5 to 2 weeks. Common diagnoses and types of patients include ortho, strokes, joints, spinal injuries, and speech.
