Job Summary:

We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality patient care and case management for patients receiving home health, hospice, and palliative care services. The RN will utilize the nursing process and individualized Plan of Care to assess, plan, implement, and evaluate patient care needs in accordance with physician orders and organizational policies.

Key Responsibilities:
  • Provide skilled nursing care to patients requiring intermittent professional nursing services in home health, hospice, and palliative care settings.
  • Conduct initial patient evaluation visits and perform ongoing reassessments of patient needs.
  • Develop, implement, and revise individualized Plan of Care based on patient condition and clinical judgment.
  • Deliver specialized nursing treatments and interventions as required.
  • Initiate preventive, restorative, and rehabilitative nursing procedures.
  • Prepare accurate clinical documentation, progress notes, and patient records in a timely manner.
  • Coordinate care with physicians, interdisciplinary team members, and other healthcare personnel.
  • Communicate changes in patient condition and care needs to physicians and appropriate staff.
  • Educate and counsel patients and families regarding care plans, treatments, and available resources.
  • Participate in staff meetings, in-service training, and quality improvement initiatives.
  • Supervise, train, and support nursing assistants and other clinical staff as needed.
