Job Title: RN HH Case Manager

Location: Oak Brook, Illinois

Pay: $3,486.80 weekly

Shifts: Monday - Friday, every 3rd weekend

Seven Healthcare are seeking an experienced RN HH Case Manager to join a dedicated home health team in Oak Brook, IL. This travel assignment offers an exciting opportunity to work in a home health environment, providing care for patients with chronic diseases and complex healthcare needs.

Key Responsibilities of the RN HH Case Manager
  • Manage a caseload of 6 visits per day, adjusting for Oasis visits
  • Provide care for patients with chronic disease management, foley catheters, wounds, PICC/infusion, and LVAD
  • Perform wound care/management and central/PICC line dressing changes
  • Work collaboratively with an interdisciplinary team
  • Schedule own active patients and adapt to changing coverage territories
  • Participate in 8 a.m. morning huddles remotely and bi-monthly team meetings in person

Requirements of the RN HH Case Manager
  • AHA BLS certification
  • Valid driver's license, insurance, and reliable vehicle
  • Minimum of 2 years of RN experience, with at least 1 year in home health
  • Proficiency in EPIC EMR system
  • Experience with wound care and management, foleys, PICC/infusion
