Job Title: Diabetes Care Registered Nurse Profession: Nursing Specialty: Diabetes Care Duration: 13 weeks Shift: Day shift with evening and weekend coverage required Hours per Shift: 8 hours Experience: Minimum years of relevant experience in diabetes care and education License: Current and valid Registered Nurse License Certifications: Certification in Diabetes Education Must-Have: Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN) Description: The Registered Nurse in the Client of Excellence plays a pivotal role in the holistic management and education of patients with diabetes. This role involves understanding the complexities of diabetes management, from medication familiarization to technical troubleshooting of devices. The RN ensures that patients receive optimal and personalized care in collaboration with the entire diabetes care team. Possesses comprehensive knowledge of all diabetes medications to counsel patients on proper administration techniques, potential side effects, and expected outcomes. Prepares prior authorizations for diabetes-related medications for insurance companies. Leads the process to appeal denials of medications to ensure patients have timely access to vital treatments. Works with pharmacies to solve coverage issues so patients have safe and effective substitutions of medications where appropriate. Educates patients on the fundamentals of diabetes, ensuring a comprehensive understanding of the disease from both a biological and practical management perspective. Fosters patient-centered approaches and tailors teaching methods to individual needs to ensure patients grasp the intricacies of managing their condition. Conducts glucose tolerance tests and ensures patient safety and accurate results. Offers culturally sensitive nutritional counseling including dietary approaches such as the Mediterranean diet. Provides patient-specific exercise counseling tailored to individual needs and limitations. Instructs patients on the implementation, usage, and troubleshooting of continuous glucose monitors and integrates devices with mobile technology. Facilitates data sharing with the center for remote consultations. Uploads data from patient home monitoring devices so that the information is available to the clinical team during visits. Teaches insulin administration, management of high and low blood sugar, sick day rules, and glucagon pen usage. Provides in-depth training on different continuous glucose monitor brands to ensure patient proficiency. Troubleshoots issues related to insulin pumps and coordinates with distributors for pump authorization while providing technical and emotional support. Ensures patients have adequate supplies on an ongoing basis to avoid interruptions in care. Educates patients on insulin pump usage, features, and maintenance. Responds to medical advice inquiries regarding diabetes management. Collaborates with the diabetes care team to identify diabetes distress and ensure timely referrals. Works alongside the registered dietitian and health coach to ensure a comprehensive approach to patient care. Upholds and promotes excellence in diabetes care in partnership with endocrinologists and nurse practitioners. This position requires outpatient experience and comprehensive knowledge of diabetes management. Must be available for a schedule that includes evening sessions and weekends as needed.
