Job Title: Nurse Coordinator Profession: Registered Nurse Specialty: Mechanical Circulatory Support Duration: 24 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 2 years cardiac ICU/CTICU or 2 years ambulatory VAD management required. License: Current Registered Nurse License Certifications: AHA BLS, ACLS Must-Have: Cardiology and ICU experience, experience managing VAD outpatient preferred. Description: The Nurse Coordinator will be responsible for coordinating the care of patients in the outpatient setting. The role involves working closely with advanced heart failure physicians, APPs, case management, social workers, and other multidisciplinary teams. Primary duties include the review of lab results and medications in collaboration with the APP/physician. The coordinator will manage outpatient referrals and appointments, conduct pre-VAD outpatient workups, including diagnostic appointments and procedures. Additionally, the Nurse Coordinator will meet with potential VAD patients to discuss VAD device physiology and lifestyle changes. The position requires the ability to float to other facilities if needed. Experience in managing VAD parameters, alarm recognition, device operation, and discharge planning is essential. The role also encompasses psychosocial assessments and monitoring for signs of complications. Knowledge of emergency procedures related to device failures is required. Candidates must demonstrate proficiency in patient education and care coordination. Ability to maintain VAD logs and submit required reports to regulatory bodies is expected.
