GENERAL SUMMARY OF DUTIES: The School Nurse provides clinical services at School Based Health Clinics as designated during school hours and transitions to the designated Community Health Center after school and on non-school days. The Pediatric Registered Nurse is responsible for ensuring the delivery of quality healthcare services at the assigned clinic site(s) to include the School Based Health Clinics, under the direction of the Site Manager (SM), Director of Nursing (DON) and Pediatric Provider. Participates in the patient's care plan by utilizing the nursing process to carry out interventions to improve the patient's health outcomes.
Follows and adheres to all federal, state, and local regulations. Complies with the standard of care, adhering to InclusivCare's guidelines, protocols, policies, and procedures, including certifying and accrediting agencies and bodies. Acts as a liaison between clinical and non-clinical staff members, providers, patients, hospitals, and other external health care agencies.

ESSENTIAL FUNCTIONS:
Assist the providers with clinic visits, conducting the following duties and tasks according to current practice standards within the RN's Scope of Practice:
Obtaining accurate vital signs, focused precise intakes, and assessments.
Conducting Point of Care Testing (POCT).
Conducting daily Quality Controls (QC), temperature checks and logs.
Administer medication utilizing the 10 medication rights to reduce the potential for errors.
Setting up and assisting with ordered procedures.
Conducting patient Care Management.
Management and adherence to VFC and LINKS program standards and procedures
Documents with meticulous attention to detail in the Electronic Medical Record (EMR).
Notifies the provider immediately of observed changes in the patient's medical condition.
Provide age-specific, quality, patient-centered care with sensitivity and respect for the diversity of human experience. Incorporating the patient and family's unique differences such as cultural, spiritual beliefs, race, ethnicity, lifestyle, socioeconomic status, age, and values into their individualized direct plan of care.
Promotes and models of Customer Service Excellence.
Represents the organization in a positive and professional manner at times.
Adjust to the clinic's fast-paced environment by accommodating unforeseen patient/clinical activity.
Communicates pertinent information and notifies the SM and DON of clinic issues that may arise immediately.
Facilitates the clinic's morning meetings and afternoon huddles.
Interacts with the medical and non-clinical staff members to deliver quality patient care.
Maintains current clinical knowledge, demonstrating competency and functions as a clinical resource.
Seeks professional growth by participating in Continuing Education (CE) activities to maintain and enhance clinical knowledge and technical skills annually.
Practices and maintains all federal, state, and local regulations, guidelines, protocols, and standards of care ensuring compliance and continuity of patient care.
Understands and consistently complies with the organization's policies and procedures.
Responsible for the clinic's ongoing Environment of Care, Infection Control, Quality and Process Improvement processes.
Prepares and completes necessary reports and documents, thoroughly and accurately, in a timely manner.
Participates in committees and task forces; and attends professional meetings and training, as required.
Works in collaboration with Director of Nursing and Pediatric Provider to develop and implement the following:

Operational Policies and Procedures
Organizational and Community Specific Projects
Supports current organizational initiatives, such as Emergency Preparedness, exhibits creativity and flexibility in times of change.
Travels to any InclusivCare location as assigned and/or needed.
Participate in community outreach activities as needed.
Perform other duties and tasks as assigned and/or needed
QUALIFICATIONS:
EDUCATION: Registered Nurse with successful completion of an approved and accredited Nursing program.
