General Purpose of Job:

Patient Placement Coordinators coordinate and optimize patient flow to promote patients' access to care, quality, and service excellence. They combine clinical, business and regulatory knowledge and skill to deliver a seamless end-to-end patient flow process while reducing significant financial risk and exposure involving medical necessity of admission processes. Through continuous assessment, problem identification and education, they facilitate the quality of health care delivery in the most cost effective and efficient manner.

Essential Duties:

This job description is intended to cover the minimum essential duties assigned on a regular basis. Team members may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.

  • Function as primary coordinator for incoming transfer requests from other facilities, which includes:

  • Confirm and document key clinical information and identify admitting service

  • Coordinate and facilitate communication between providers

  • Provide assessment and communicate changes in patients' clinical condition for proper bed placement

  • Efficiently and consistently prioritize patients accepted to University of Michigan Health-Sparrow Lansing

  • Function as primary coordinator for in-house emergency department admissions, which includes:

  • Identify attending physicians for patients being admitted

  • Conduct clinical reviews of the patient charts to determine appropriate financial class for patient admissions (i.e., inpatient versus observation status)

  • Coordinate and facilitate communication between providers

  • Function as primary bed placement coordinator for patients admitted to the hospital, which includes:

  • Assign admissions from Emergency Department, Post Anesthesia Care Unit, other internal departments, physician offices, outlying hospitals, etc.

  • Assign admitted patients to the most appropriate level of care and assist with special needs requests related to bed assignment, such as critical care, telemetry, isolation, etc.

  • Prioritize bed cleaning as needed for efficient patient flow

  • Provide service excellence and effective communication with team members, other departments, and external customers

  • Participate in departmental Performance Improvement, Corporate Compliance, Patient Satisfaction, and other activities to ensure optimal operations of the department. Actively participates in the design and implementation of strategies to improve department operations and patient outcomes.

  • Attend and actively participate in throughput meetings to plan for potential and known admissions, discharges, and transfers (as needed).

Job Requirements

General Requirements • Registered Nurse with current Michigan license. Work Experience • Minimum of two years recent acute care hospital RN experience within the last seven years (i.e. emergency department, critical care, intermediate, or medical/surgical units), or two years previous experience in an Admission/Bed Placement Center at an acute care hospital of similar size and volume of admissions. Education • Graduate of an accredited Nursing program • Bachelor's degree in nursing preferred Specialized Knowledge and Skills • Demonstrates excellent clinical knowledge, skills and judgment, with strong clinical nursing assessment skills. • Demonstrates excellent critical thinking skills. • Demonstrates ability to handle multiple tasks concurrently, think logically and sequentially, creatively solve problems. • Demonstrates ability to quickly adjust to change. • Demonstrates ability to work independently, organize and prioritize work to meet all deadlines. • Demonstrates excellent verbal and written communication skills, including relationship building, negotiation, persuasion, marketing and advocacy. • Demonstrates ability to work in stressful situations, manage conflict and assume a leadership role. • Proficient in use of computers. • Minimum typing speed of 40 words per minute • Achieves a score of 85% or higher on a customer service test
