JobPurposeSummary:Responsible for the planning, implementation and evaluation of all nursing care prescribed and provided to the resident. Provides prescribed medical treatments and care to the residents in the nursing community by performing the following duties:Providesservicesinaccordance withnursepracticeact,state/federal regulations and nursingcommunity policies and procedures.Performs anddocumentsresidentassessments onadmission and onacontinuing basis,as necessary.Monitors anddocuments changeinhealthstatusthrough continuing assessment ofthe resident.Reviews/revisesassessments andcareplansatadmission, periodically perregulation, and atchange in resident physical, mental or psychosocialcondition.Observes resident care needs, initiates nursing interventions and evaluates outcomes. Report resident physical andpsychosocial status tonursingstaff,physicians, resident''s families and other departments, as appropriate.Transcribesand carriesoutphysician''sorders,asappropriate.Administers medications, asprescribed byaphysician.Monitors residentfortherapeutic results and side effects.Administerstreatmentsandspecialproceduresasprescribedbyaphysician.Completes allmedical recorddocumentation asrequired byaccepted standards ofnursing practice, federal and state guidelines.Assistsphysicianasnecessaryduringrounds andexaminationortreatmentofresidents.Activelyparticipatesinthecareplan process.Assistsresidentswithactivitiesofdailyliving,asnecessary.Assists residents with meeting their nutritional/hydration needs.Assists the residents regarding regaining lost abilities and to maximize their strengths, to assist in creating an atmosphere of independence.Superviseandparticipateinresidentprogramming.Maintainconfidentialityofallresident''sprivatehealthinformation.Offersemotionalsupporttoresidents'families,e.g.,duringadmissionadjustment,the griefprocess,copingwithchronicillnessandworkingwiththecognitively impaired resident and his/her family.Actsasateamleader.Directsandsupervises tasksofnon-professionalstafftoassure quality of nursing care is maintained.Assistswiththeevaluationofstaff.Initiatesdisciplinaryaction,asneeded.Participates incommunity in-service programs by attendingscheduled in-services, suggestingin-servicetopics,teachingotherstaffonaninformalandformalbasis,as required.Organizesworkdayaccordingtoacceptedroutinesandprioritizesactivities.UnderstandsandimplementstheFireSafetyandDisasterPreparednessPlanand evacuation procedures of thecommunity during drills and emergencies.Followallsafety,security,infection control, Universal PrecautionsandHazardous Materialpoliciesandprocedures.Performsalltaskstoassureresidentandpersonal safety and theprotection ofco-workers and community property.Interactswithresidents, guestsandstaffinanatmosphereofhospitality.Performotherdutiesasassigned.Qualifications:Toperformthejobsuccessfully, anindividual mustbeabletoperformeachessential duty satisfactorily.Therequirements belowarerepresentativeoftheknowledge andskillsand abilities required.Reasonable accommodationsmay bemade toenable individuals with disabilities to perform the essential functions.EducationRequirementsandExperience:Mustbeagraduate ofanaccreditedSchoolofNursing.Must have,asaminimum, one(1)yearofexperience asasupervisorinahospital, nursing care facility, or other related health care facility.Mustpossessacurrent,unencumbered,activelicensetopracticeasaRegistered Nursein West Virginia.Mustbeableread,write,speakandunderstandtheEnglishlanguage.BasicLifeSupportCertification (CPR)orwillingnesstoobtainKnowledgeandCompetencies:Must havebasicknowledge ofcomputers.Knowledge ofnursing and medical practices and procedures, aswell aslawsand state/federal regulations concerning dementia care andskilled nursingcare inWest Virginia.Knowledge oftheconcepts ofagingandconcerns oftheelderly.
