Registered Nurse - Hospice

A-Line Staffing is now hiring Registered Nurse - Hospice in Morristown, NJ 07920. The Registered Nurse - Hospice would be working for a Major Healthcare System and has career growth potential.

Registered Nurse - HospiceHighlights:

• Shift Options:

1. Monday???Friday, 8:00 AM???4:00 PM; on-call/weekends required (1 weekend per month)

2. 11a-7p and every other weekend
• Pay Range: $60-65 Hourly, plus mileage reimbursement for travel to different counties to visit patients as needed

Registered Nurse - Hospice Responsibilities:

• Perform patient assessments to determine appropriateness for hospice care; identify learning needs and progress toward goals
• Develop, implement, and revise patient/family plan of care; prioritize care needs
• Complete admission/discharge assessments and documentation within required timeframes; transfer data twice daily
• Provide age-appropriate teaching to patients/families and document education provided
• Provide case management including interdisciplinary communication and participation in IDT meetings (every 2 weeks)
• Conduct HHA supervisions per standards; follow scheduling process and productivity standards
• Maintain safe environment; follow infection control practices
• Medication management, including routine medication reconciliation and maintaining current medication orders
• Participate in performance improvement/UR activities as requested; support orientation/mentorship and cross-training

Registered Nurse - Hospice Qualifications:

• Current New Jersey RN License (ASN or Diploma accepted)
• BLS required
• Valid New Jersey driver???s license required
• 2+ years RN experience in a home hospice environment (3+ years preferred)
• EMR experience; strong critical thinking and communication skills
