Innovative Wound Solutions, a leading healthcare organization dedicated to providing innovative and patient-centered care, is seeking a highly motivated and experienced Nurse Practitioner to work within our established Mobile Wound Care Program. Position Overview:
We are looking for a dynamic and autonomous Nurse Practitioner to serve the Phoenix location. The successful candidate will play a crucial role in providing direct patient care, developing chronic care plans, and collaborating with referring providers to optimize patient outcomes.
Responsibilities:
- Clinical Leadership:
- Provide direct patient care through all available channels and products.
- Conduct comprehensive assessments, develop treatment plans, and implement evidence-based care practices.
- Monitor and evaluate patient progress, adjusting treatment plans as needed.
- Collaborate with interdisciplinary teams to ensure seamless patient care coordination.
- Chronic Wound Care Planning and Management:
- Develop personalized treatment plans for patients with wounds, focusing on long-term management and prevention of complications.
- Educate patients and families on self-management strategies and lifestyle modifications.
- Communication and Collaboration:
- Establish and maintain effective communication channels with referring providers to ensure continuity of care.
- Collaborate with healthcare professionals, including physicians, nurses, and other specialists, to optimize patient care.
Qualifications:
- Current and unrestricted Nurse Practitioner license in Arizona.
- Certification as a Nurse Practitioner with a specialization in wound care management is preferred.
- Minimum of 1 year of experience in wound care.
- Strong interpersonal and communication skills.
