Nurse Case Manager

A-Line Staffing is now hiring a Full-Time, Field Based, Long Term Care Manager in New Castle County, DE! (THIS IS A POSITION COVERING NEW CASTLE COUNTY. DOING AT HOME VISITS. PAY IS $37/HR WITH MILEAGE REIMBURSEMENT).

We are seeking an LTSS Service Care Manager! This role focuses on conducting Long-Term Services and Supports (LTSS) Level of Care redeterminations, care coordination, and transition of care activities for members.

The Service Care Manager will perform a combination of in-home member visits and case management support functions, working closely with the LTSS care management team to ensure members remain compliant with required assessments and receive appropriate services.

This position plays a critical role in supporting a rapidly growing membership population and ensuring timely, high-quality care management.

Long Term Care Manager Compensation

• The pay for this position is $37.00/hr + mileage reimbursement
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Long Term Care Manager Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required availability for this position is Monday – Friday 8am – 5pm

• Long Term Care Manager Responsibilities

  ·         Conduct LTSS Level of Care redeterminations

  ·         Complete member home visits and assessments

  ·         Support transition of care activities

  ·         Assist LTSS case managers with caseload management

  ·         Evaluate members’ functional status, especially older adults and individuals with disabilities

  ·         Perform medication reconciliation as part of care coordination

  ·         Document assessments and ensure compliance with program requirements

  ·         Collaborate with care teams to support member needs and care planning

  Typical Day

  • Monday–Friday schedule, 8:00 AM – 5:00 PM (no overtime expected)
  • Travel to member homes for in-person assessments
  • Conduct Level of Care evaluations and care coordination activities
  • Coordinate with internal LTSS team members throughout the day
  • Manage assessments and documentation tasks between visits

  Long Term Care Manager Requirements

  Education & Experience

  • Required: Bachelor’s degree
  • Required: 2–4 years of related experience

  Licensure

  • Required: Delaware or Compact RN License
  • Required: Valid driver’s license

  Must-Have Qualifications

  • Experience with Level of Care redeterminations
  • Experience with transition of care and case management
  • Ability to travel and conduct home visits
  • Computer literacy
  • Experience evaluating older adults and individuals with disabilities

  Nice to Have

  • Experience as a Service Care Manager
  • Medication reconciliation experience
