Job Title: Nurse Care Manager
Job Description
The Nurse Care Manager provides professional, patient-centered nursing care in an ambulatory setting, delivering both in-person and telephonic services to patients and families. This role supports a multidisciplinary plan of care by collecting and analyzing patient data, maintaining and updating care plans, and evaluating outcomes of nursing interventions. The Nurse Care Manager documents and communicates all care activities according to professional and organizational standards, delegates tasks to appropriate licensed and unlicensed team members, and ensures compliance with regulatory requirements, including Joint Commission and Patient-Centered Medical Home (PCMH) standards.
Responsibilities
- Provide direct ambulatory nursing care and telephonic support to patients and families in alignment with the multidisciplinary plan of care and established policies and procedures.
- Facilitate individual and group nursing visits focused on chronic illness management, including patient education on disease management and treatment options.
- Identify patients at risk for poor outcomes or those requiring more intensive services, and provide enhanced outreach and frequent follow-up by phone and in person.
- Deliver complex case management services, including chronic disease case management, critical diagnosis care coordination, transition of care management, high-risk clinical tracking, complex medication management, and system utilization support.
- Conduct medication reconciliation and provide adherence counseling, assessing patient knowledge, barriers to adherence, and barriers to effective self-care.
- Triage walk-in and same-day clinic patients, determining appropriate level of care and coordinating timely interventions.
- Collaborate with a multidisciplinary care team to address physical and psychological aspects of health, supporting comprehensive, whole-person care.
- Work closely with the Transition of Care Team to promote safe and effective transitions between care settings.
- Partner with providers during patient appointments or in dedicated nursing visits to reinforce health promotion, disease prevention, and disease management education.
- Support patients in developing self-management goals, regularly evaluate progress toward these goals with the interdisciplinary team, and coordinate follow-up care as needed.
- Access and coordinate appropriate internal and external resources to meet patient and caller needs.
- Demonstrate awareness of cultural, ethnic, gender, sexual orientation, and racial differences and their potential impact on patient outcomes, integrating this awareness into care planning and communication.
- Teach patients self-care skills, including medication administration (such as self-injections, nebulizers, and inhalers), wound care, and monitoring of blood sugars and blood pressures.
- Provide education on the importance of preventive health screenings and support patients in accessing these services.
- Delegate clerical and non-nursing clinical tasks to assistive personnel appropriately, ensuring safe and effective task distribution.
- Utilize interpreter services to ensure clear and respectful communication with patients who require language assistance.
- Attend nursing departmental and site staff meetings and participate in committee meetings as requested to support quality improvement and care coordination initiatives.
- Document incoming and outgoing calls in real time and communicate effectively with team members to ensure continuity of care.
- Record telecommunication encounters that accurately reflect the patient's actual or potential health needs and the nursing interventions provided.
- Collect health status data relevant to the patient's presenting problem in a continuous, systematic, and comprehensive manner.
- Complete review of systems and physical assessments, correlate findings with the patient's health history, and prepare patients for further examination or treatment.
- Ensure compliance with regulatory body standards, including Joint Commission and PCMH requirements, and support adherence to organizational policies and procedures.
Essential Skills
- Active Registered Nurse (RN) license in the state of Maryland.
- Graduate of an accredited school of nursing.
- Experience documenting care in an Electronic Medical Record (EMR).
- Ability to provide ambulatory nursing care, including triage, chronic disease management, and patient education.
- Strong analytical and critical thinking skills to assess patient needs, identify high-risk patients, and evaluate outcomes of interventions.
- Demonstrated ability to collaborate effectively within a multidisciplinary team and coordinate complex care.
- Proficiency in patient education, including teaching self-care skills, medication administration, and preventive health practices.
- Ability to prioritize competing responsibilities and manage a dynamic workload in a busy clinic environment.
- Flexibility and adaptability to a changing clinical environment and schedule demands.
- Effective verbal and written communication skills, including clear documentation and use of interpreter services when needed.
- CPR certification by the scheduled start date.
Additional Skills & Qualifications
- One year of nursing experience in an ambulatory or acute care setting preferred.
- Bachelor of Science in Nursing (BSN) preferred.
- Experience in case management, care management, or managed care environments.
- Background or experience in OB/GYN, labor and delivery, or women's health services.
- Experience with complex case management, utilization management, or navigation within healthcare systems.
- Familiarity with electronic health records and comfort working with computer-based word processing and database applications.
- Experience working with uninsured, low-income, or historically underserved populations.
- Significant related experience with the populations served may substitute for certain educational preferences.
- Demonstrated cultural competence and sensitivity to diverse patient backgrounds.
- Experience in patient referrals, care coordination, and navigating community and healthcare resources.
Work Environment
This role is based in an ambulatory care center located in the heart of the Mt. Vernon neighborhood of Baltimore, within a historic downtown building. The site offers Adult Primary Care, OB/GYN, and additional specialty services, providing broad exposure to multiple clinical specialties and a diverse patient population, including uninsured and low-income community members. The work schedule follows standard weekday hours, Monday through Friday, with no weekend shifts, supporting work-life balance. The environment is team-oriented and mission-driven, with a strong focus on comprehensive, full-service care and continuity across the care continuum. Staff use Electronic Medical Records and standard clinical equipment typical of ambulatory primary care and OB/GYN settings. The culture emphasizes collaboration, patient-centered care, and continuous learning, with opportunities to participate in committees and quality initiatives and potential for transition-to-hire. Professional, clinic-appropriate attire is expected in keeping with an urban outpatient healthcare setting.

Pay and Benefits
The pay range for this position is $40.00 - $45.00/hr.
