POSITION SUMMARY
The Licensed Practical Nurse is responsible for providing direct and indirect nursing care to patients and families in the clinic under the direction and supervision of the Nurse Manager and Director of Nursing DON. The LPN is expected to work well with every member of the clinical team to provide safe, high-quality, and efficient primary care to a patient population diverse in age, health status, and socio-economic level. This position requires strong team work and communication skills with the care team.
MINIMAL QUALIFICATIONS
Education Graduate of an accredited School of Nursing
Licensure Florida license
BLS certification
Computer literacy
Experience in Community Health Preferred
RESPONSIBILTIES AND PERFORMANCE EXPECTATIONS

• Be familiar with CFHCs program philosophies, goals, and objectives. • Be cognizant of and comply with all CFHCs policies and procedures as well as state and federal regulations. • Be competent in performing the duties of a nurse. • Understand the requirements of CFHCs Exposure Control Plan. • Personally utilize universal precautions when practicing in a clinical setting. • Participate in staff outreach and off-site health care programs as requested by the DON, Chief Medical Officer CMO, represent CFHC in the community. • Report as needed to DON any pertinent information or situations that impact on patient care or CFHC liability. • Attend and participate in mandatory CFHC meetings, Center Specific meetings, Corporate meetings, and other meetings as designated by the DON or CMO. • Demonstrate ability to work cooperatively with other members of the clinical team. • Be supportive of coworkers. • Maintain a neat and professional appearance at all times. • Carry out any other activities deemed appropriate by the DON or CMO.
Screening and Assessment
• Screen patients according to CFHCs protocols, example obtaining height, weight, vital signs, vision, hearing testing, and perform any other procedures directed by the provider. • Accurately obtain patients medical history. • Perform accurate charting in patients medical records in accordance with CFHCs policies and procedures. • Perform waived lab analysis, ex urinalysis, pregnancy test, hemoglobin, glucose, strep, flu. • Administer injections according to position requirements, example subcutaneous, intramuscular, and intradermal injections. • Prepare and administer vaccines and medications as directed by the provider. • Perform procedures as ordered, ex nebulizer treatments, ear lavage. • Counsel patients per treatment plan, ex diet, exercise, medication.
Emergency Protocols and Infection Control
• Understand all emergency protocols and respond accordingly. • Maintain exam rooms and work areas, ex keeping rooms well-stocked, orderly, and clean to include disinfecting exam tables between patients as per infection control policy. • Maintain strict infection control, ex sterilizing instruments, performing procedures, needle disposal.
Scheduling and Referrals
• Accurately make, cancel, and reschedule appointments in the computer appointment system. • Follow-up on appointment to determine patient compliance and contact patients to reschedule no-show policy. • Understand and assist in the referral process of patients to outside services or providers, follow-up on patient referrals.
Clinical Logs and Communication
• Complete and ensure accuracy of clinical logs, ex hospital temperature, medication, vaccine administration. • Maintain patients concerns within electronic medical records EHR. • Communicate with patients via telephone, the patient portal, and other modalities. • Keep providers and other team members updated regarding clinical changes.
