Overview:
Under the supervision of a RN and in accordance with agency policies and state practice, the Licensed Practical Nurse provides nursing services to individuals and families in their residence under the supervision of a registered nurse.
Responsibilities:
- Performs prescribed individualized patient care to patients as delegated.
- Participates in patient case conferences as appropriate; is an active member of the interdisciplinary team.
- Assists the RN in the preparation, evaluation and modification of the patient's plan of care as appropriate.
- Observes, documents and reports to the RN as appropriate, the patient's physical and emotional needs and reactions to care.
Qualifications:
- Must be a Licensed Practical Nurse in Vermont and New Hampshire or hold a compact license.
- Graduate of an accredited school as a Licensed Practical Nurse
- One year of experience as an LPN in a home health care setting is preferred
Required Licensure/Certifications:
- Unencumbered LPN license in VT and NH or compact.
