A-Line Staffing is now hiring a Full-Time, Remote, Home Nurse Case Manager in New Castle County! (THIS ROLE IS A MIX OF MEMBER HOME VIISTS AND HYBRID ADMIN WORK).

Job Summary

The LTSS Service Care Manager supports the Long-Term Services and Supports (LTSS) program by completing level of care redeterminations, transition of care activities, and ongoing case management duties. This role is critical in ensuring members maintain eligibility and receive appropriate services while supporting an increasing membership population.

The position includes a blend of home-based member visits and coordination of care activities in support of the broader LTSS case management team.

Home Nurse Case Manager Compensation

• The pay for this position is $36.00/hr + mileage reimbursement
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Home Nurse Case Manager Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required availability for this position is Monday-Friday 8am-5pm

• Home Nurse Case Manager Responsibilities

  ·         Conduct scheduled home visits with members

  ·         Perform level of care redeterminations

  ·         Support transition of care processes

  ·         Assist LTSS case managers with caseload overflow

  ·         Document assessments and ensure compliance requirements are met

  ·         Maintain consistent communication with members and care teams

  Home Nurse Case Manager Requirements

  ·         Bachelor’s degree

  ·         2–4 years of related experience

  ·         Active Delaware or Compact RN License

  ·         Valid Driver’s License (required for home visits)

  ·         Experience in:

  o   Level of Care Redetermination

  • • Transition of Care
    • Case Management

  ·         Ability to work independently in a home-based workspace

  ·         Computer proficiency
