Job Overview – Home Infusion Pharmacist
Compensation: $125,000 – $145,000/year
Location: Palm Beach County, FL
Schedule: Monday to Friday (On-Site)

TAG MedStaffing is hiring a Home Infusion Pharmacist in Palm Beach County, FL for our client, supporting infusion pharmacy operations, medication management, sterile compounding oversight, and patient care services within an alternate care setting. This role will collaborate closely with physicians, nurses, patients, and caregivers to ensure safe, effective, and compliant medication therapy management. The ideal candidate will bring strong clinical pharmacy expertise, home infusion or acute care experience, and a commitment to delivering high-quality patient care.

Responsibilities as the Home Infusion Pharmacist:
  • Clinical Pharmacy Services: Review patient therapies, assess treatment appropriateness, provide dosing recommendations, and monitor medication outcomes.
  • Medication Management: Process prescription orders, maintain patient medication profiles, and oversee medication dispensing activities.
  • Sterile Compounding Oversight: Ensure compliance with USP standards, pharmacy regulations, quality assurance procedures, and sterile compounding requirements.
  • Patient Education: Counsel patients and caregivers regarding medication administration, infusion therapies, treatment expectations, and adherence.
  • Interdisciplinary Collaboration: Partner with physicians, nurses, dietitians, and healthcare providers to optimize patient care and clinical outcomes.
  • Pharmacy Operations: Support pharmacy workflow management, supervise support personnel, oversee documentation requirements, and contribute to operational efficiency.

Qualifications for the Home Infusion Pharmacist:
  • Education: Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program is required.
  • Licensure: Active Registered Pharmacist license in the state of employment is required.
  • Experience: Minimum 2 years of acute care pharmacy experience or 1 year of home infusion pharmacy experience is required.
  • Industry Knowledge: Strong knowledge of intravenous therapy, sterile compounding, pharmacokinetics, infusion services, and medication therapy management is required.
  • Technical Skills: Proficiency with pharmacy management systems, electronic health records, Microsoft Office applications, and clinical documentation platforms is required.
  • Skills & Attributes: Strong clinical judgment, communication, organizational, leadership, and problem-solving skills with the ability to manage multiple priorities in a patient-focused environment are required.
