What You'll Do

• Leading and evolving a compliant, high-performing Care Management program aligned with regulatory standards.
• Coaching and mentoring a tenured team (including 3 Case Managers, 1 PRN, and a Clerk) with clarity, accountability, and professional development in mind.
• Serving as a trusted liaison across departments and with community partners to support safe transitions of care.
• Participating in LOS meetings and Patient Care Conferences to ensure efficient, patient-centered outcomes.
• Reinforcing standards and practices that support regulatory compliance and consistent performance.

Requirements:

• RN and Bachelor’s required; Master’s preferred
• 3+ years of leadership in Case/Care Management with Utilization Review experience (hospital experience required)
• Case Management Certification strongly preferred
