About the Role

We are seeking a dynamic and relationship-driven Clinical Liaison to support patient referrals and admissions to Schwab Rehabilitation Hospital. In this role, you'll blend clinical expertise with outreach and coordination-building partnerships, evaluating patients, and ensuring seamless transitions into care.

This is an ideal opportunity for a clinician who enjoys autonomy, collaboration, and making a direct impact on patient access to rehabilitation services.

What You'll Do

• Build and maintain strong relationships with referral sources (case managers, social workers, physicians, etc.)
• Conduct on-site and remote patient assessments to determine rehab eligibility
• Coordinate admissions and ensure timely patient transitions
• Educate patients, families, and referral partners on acute rehabilitation services
• Manage multiple referrals and provide admission decisions within 24 hours
• Ensure accurate and compliant documentation and physician approvals
• Identify new referral opportunities and grow referral networks
• Collaborate with internal teams to achieve census goals

What We're Looking For

• Degree in Nursing, Social Work, PT, OT, Speech Therapy, or Athletic Training
• Active Illinois licensure required
• Experience in post-acute care or clinical liaison role preferred
• Strong communication, multitasking, and decision-making skills
• Ability to work independently and manage a fast-paced workload
• Experience with EPIC or MEDITECH (preferred)
• Valid driver's license
