You'll develop and strengthen relationships with hospitals, physicians, case managers, social workers, discharge planners, and other referral partners while helping patients and families access the compassionate end-of-life care they deserve. As a Clinical Liaison, you will be the bridge between healthcare providers, patients, families, and our interdisciplinary hospice team. You'll educate referral sources about hospice services, coordinate seamless transitions of care, and advocate for patients facing serious illness. What You'll Do
- Develop and maintain strong relationships with hospitals, physician practices, skilled nursing facilities, and community referral sources.
- Educate healthcare professionals, patients, families, and community organizations about hospice services and eligibility.
- Identify and cultivate new referral opportunities throughout the service area.
- Coordinate patient referrals and facilitate smooth transitions into hospice care.
- Collaborate with interdisciplinary teams to ensure continuity of care and exceptional patient experiences.
- Represent Georgia Hospice Care at community events, healthcare meetings, and networking opportunities.
- Maintain accurate documentation and communication regarding referrals and admissions.
- Serve as a compassionate resource and advocate for patients and families exploring hospice services.

Qualifications:
A heart to serve patients and families and a passion for providing the best possible care
- Education: Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program, OR Graduate of an accredited school of nursing (LPN OR RN).
- Licensure: Current LBSW, LMSW, or LCSW licensure in the state of practice preferred, where applicable. Registered Nurse or Licensed Practical Nurse in the state. Must successfully perform and comply with all credentialing as required by hospital/healthcare facility.
- Experience: 2 years' experience in a clinical care setting or hospice preferred. Experience building professional relationships with healthcare providers and community organizations strongly preferred.

- Passion for serving patients and families with empathy and professionalism.
- Knowledge of healthcare systems, community resources, and end-of-life care preferred.

- Strong organizational and problem-solving skills.
- Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100lbs and bear the weight of an average adult effectively. Must meet all hospital and healthcare facility credentialing requirements.
