launching an innovative, largely virtual outpatient care model designed for low-income
people living with uncontrolled chronic conditions (starting with people with diabetes)
who also have high social and behavioral needs (whole person care), and we are

The Opportunity:


The Clinical Care Manager will lead multiple outpatient multidisciplinary care teams
composed of Community Health Workers, Behavioral Health Clinicians, and Registered
Dietitians. This role also involves working directly with external providers such as PCPs
and NPs to ensure holistic, seamless, high-quality care for our diverse patient
population.


The Clinical Care Manager is responsible for improving both patient care and the
patient experience while managing and supporting the care team’s operations. This is a
largely virtual outpatient multidisciplinary care model that we are learning to scale,
and this role must be able to thrive working in a fast-paced, often ambiguous,
high-patient-touch outpatient environment that includes phone calls, text messages,
virtual visits, and community visits with operational elements still being defined. This
role requires compassion, creativity, innovative problem-solving, and the ability to work
collaboratively with the care teams to ensure high-quality, patient-centered care.

 

Responsibilities:


Clinical Support & Coordination:


Oversee the daily activities of assigned multidisciplinary care teams,
ensuring effective collaboration and seamless care delivery.
Provide clinical expertise and support to team members, particularly in
managing complex cases and clinical decisions.
Lead clinical triaging efforts, ensuring appropriate escalation to maintain
patient safety and care quality.
Enhance the clinical effectiveness of the teams, ensuring adherence to the
Vayu Care Model and best practices.
Ensure adherence to HIPAA compliance regulations to protect patient
privacy and confidentiality.
Team Management & Development:
Oversee all aspects of team management, including recruitment,
onboarding, training, coaching, performance evaluations, disciplinary
actions, and team transitions while fostering a collaborative,
high-performing environment focused on delivering exceptional patient
care.
Actively engage with your team in daily operations, collaborating closely
with them to solve challenges, provide clinical guidance, and ensure
efficient workflows.
Create an environment that encourages continuous learning, professional
development, and open communication within the care team.
Build and maintain strong working relationships with external clinic staff,
community clinics/federally qualified health centers (FQHCs), and providers
to support a collaborative, patient-centered approach to care.
Facilitate regular huddles and case conferences to address challenges and
improve team morale and productivity.

Operational Excellence:

Help optimize workflows, schedules, and clinical documentation to improve
team effectiveness and ensure efficient, seamless patient care.
Help monitor and improve key clinical effectiveness metrics, and develop
actionable plans to address any performance gaps.
Ensure compliance with HIPAA, regulatory standards, and clinical care
contracts, particularly as they pertain to California regulations.

Problem-solve and multitask effectively to adapt effective human centered
care delivery to meet the evolving needs of both patients and the care
team.

Strategic Growth & Continuous Improvement:

Contribute to the development and implementation of clinical projects that
drive improvement in both patient care and team performance.
Support the adoption of technology and EMR systems that enhance care
delivery and streamline workflows as well as be a superuser.
Identify opportunities to adjust team roles and responsibilities to ensure
continued alignment with Vayu Health’s care model and strategic goals as
we grow.
Help implement and recruit members into our program.
Performs other duties as assigned.


Required Qualifications:

Bachelor of Science in Nursing (BSN).
Active and unencumbered RN license in the state of California.
Active CDCES certification OR will be eligible to take the CDCES exam
within 1 year of employment.
Experience working with underserved populations.
Minimum of 5 years of clinical nursing experience, with demonstrated
proficiency in triaging and managing complex patient cases.
Minimum of 3 years of comprehensive supervisory experience,
showcasing the ability to effectively lead, mentor, and motivate a
multidisciplinary clinical team to achieve high-performance standards.
In-depth knowledge of regulatory compliance in healthcare, with a
focus on California regulations.
Excellent communication, conflict resolution, and team-building skills,
with a commitment to fostering a collaborative and patient-centered
work environment.

Preferred Qualifications:

Fluency in both English and Spanish
Lived experience
Experience in design and/or improvement work
Experience volunteering in your community
Known for thinking outside the box to improve member care
