Portneuf Medical Center is a nationally recognized healthcare leader with 205 beds, serving as the regions tertiary care and Level II Trauma Center, housing the Portneuf Heart and Vascular Institute, Portneuf Cancer Center, Portneuf Medical Group, and Portneuf Air Rescue.
Responsibilities
To assure clinical appropriateness of services and efficient use of resources through collaboration with the entire healthcare team for acute care patients. The Case Manager is responsible for medical necessity review, discharge planning, and utilization review.
Performs medical necessity reviews and initial assessment within 24 hours of patient admission.
Identifies and communicates changes in the patients plan of care to the healthcare team.
Maintains detailed knowledge of community resources and processes for referrals.
Completes all assigned UR tasks on a daily basis.
Using LOS anticipates patient needs, stewards resources, and timely transitions patients to alternative levels of care.
Documents in patient records in a consistent and timely manner to include initial screenings and discharge plans.
Qualifications
Job Requirements:
Current RN license
BLS certification must be obtained within 14 days of hire or transfer into the role and prior to
