Behavioral Transfer Care Coordinator

Job Summary and Responsibilities

• Coordinates care with other health care community providers.
• Promote patient retention by providing focus on getting patients to the right service with the right provider at the right time - one call resolution.
• Manage phone call activity to minimize any delays experienced by customers.
• Conduct intake for new referrals to quickly and accurately triage the needs of the caller.

Job Requirements

Preferred

• Bachelors degree in Social Work or Human Services related field
• Acute care experience in social work or behavioral services
